PSCO 4 gas meter detected carbon monoxide increase in the IGS room when the system was running.

PSCO identified water sprinkler nozzle actively dripping water onto an electrical box for the incinerator, and the sprinkler pipe had rags and line tied to it.

Vessel had missing float switch cover missing for boiler cascade tank. Vessel showed proof of purchase for the cover.

ESD's on deck and in cargo control room activate required alarms, but steam still flows through and turns cargo pump. Pump RPMs are not changed when ESD is activated. 33 CFR 156.170(c)(5)

Ch II-2 Reg 9.3.1 Where "A" class divisions are penetrated, such penetrations shall be tested in accordance with the Fire Test Procedures Code. Such penetrations shall be suitably insulated by extension of the insulation at hte same level of the division.
