The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. MSU Savannah received a report from the vessel that two (02) alarms from the main engine safety panel were active, "wiring abnormal" and shut down device active", which affected control of the vessel's main engine. 74SOLAS(04)I/11(a)

Electrical installations shall be such that the safety of the crew and ship from electrical hazards will be ensured. PSCO observed a 440v electrical cable installed on the emergency generator switchboard and coming out of one of the doors on the backside of the panel. Chief Engineer was not aware of how long the connection point had been installed for, but removed the installed cable in the presence of the PSCO. 74 SOLAS (04) II-1/40.1.3

ICLL I/25.3 GUARD RAILS FITTED ON SUPERSTRUCTURE AND FREEBOARD DECKS SHALL HAVE AT LEAST THREE COURSES. THE OPENING BELOW THE LOWEST COOURSE SHALL BE NOT MORE THAN 380 MM APART. IN THE CASE OF SHIPS WITH ROUNDED GUNWALES THE GUARD RAIL SUPPORTS SHALL BE PLACED ON THE FLAT OF THE DECK. IN OTHER LOCATIONS, GUARD RAILS WITH AT LEAST TWO COURSES SHALL BE FITTED. THE OUTERGUARD RAIL ON THE PORT SIDE IN THE VICINITY OF BAY 18, THE THIRD RAILING COURSE WAS WASTED, AND LOOSE.

Paint Locker and flammable liquid lockers shall be protected by an appropriate fire-extinguishing arrangement approved by the administration. Port State Control Officers observed a pallet of paint in the steering flat. In addition to paint, large amounts of refrigerant, lube and hydraulic oils, and chemical stores were observed in the space. Chemical stores were stowed outside the chemical locker and refrigerants were not secured to prevent movement at sea. SOLAS 2001 II-2/18.7 16 A/C
