ALL ACCESS OPENINGS IN BULKHEADS AT ENDS OF ENCLOSED SUPERSTUCTURES SHALL BE FITTED WITH DOORS OF STEEL OR OTHER EQUIVELENT MATERIAL, PERMENENTLY AND STRONGLY ATTACHED TO THE BULKHEAD, AND FRAMED, STIFFIENED AND FITTED SO THAT THE WHOLE STRUCTURE IS OF EQUIVELENT STRENGTH TO THE UNPIERCED BULKHEAD AND WEATHERTIGHT WHEN CLOSED. PSCO FOUND UPPER DECK PORT AND STARBOARD DOORS WERE NOT WEATHERTIGHT. DID NOT CREATE AN ENTIRE SEAL AROUND THE KNIFE EDGES. CREW WAS ABLE TO FIX PRIOR TO [name withheld].2 REG 12 (1)
