Records of daily hours of rest must be maintained onboard and each affected crewmember must endorse it.

Transfer procedures must specify the duties by title of each officer, PIC, deckhand and any other person required for each transfer operation. Master or Licensed Mate must act as PIC while procedures are amended and placed onboard. Transfer Procedures content must be IAW 33CFR155.750. Upon further review, noted that it's missing cargo info, line diagram, emptying discharge containment system, and oil-spill reporting procedures.

Fire and abandonship drills must be performed on alternate weeks. These drills have not been performed consistently, and were missed multiple times.

Paint locker must be constructed of steel and be protected with fixed firefighting equipment if dimensions exceed 60 cubic feet. Paint locker onboard is aluminum made and must be replaced or be wholly lined with steel.

Each OSV must carry the following equipment for the line-throwing appliance. For an appliance approved under 46 CFR 160.031 (shoulder gun type). One auxiliary line consisting of at least 500 ft. of 3in. circumference manila complying with federal specification T-R-605. Twenty-five cartridges of the caliber and loading specified in the instructions furnished by the manufacturer of the gun. Vessel did not have auxiliary line on board, and was missing one cartridge. - 46 CFR Part 133.170 / 46 CFR Part 160.031

Except as provided by this part, the design, installation, testing, and inspection of machinery for OSVs of less than 6,000 GT ITC must comply with subchapter F. Fluid conditioner fittings (fuel filters) must meet ASTM 1201 standards. The vessel’s port and starboard generators have nonmetallic filters that do not meet ASTM 1201 fire test standards. - 46 CFR Part 128.110 / 46 CFR Part56.15-5

Except as provided by this part, the design, installation, testing, and inspection of pressure vessels for OSVs of less than 6,000 GT ITC must comply with subchapter F. Where pressure-indicating gages are used, they shall be chosen to be compatible with the pressure to be indicated. In no case shall the upper range of the gage be less than 1.2 times nor more than 2 times the pressure at which the relieving device is set to function. Installed air receiver gauges are more than 2 times the pressure at which the relieving device is set to function. - 46 CFR Part 128.110 / 46 CFR Part 54.15-5

The frame or support of each semi-portable fire extinguisher must be secured to prevent the extinguisher from shifting in heavy weather. There was a semi-portable extinguisher on deck that didn’t have a means for securing. - 46 CFR Part 132.240

The owner of a vessel subject to inspection shall ensure that such vessel is equipped with a vessel master key control system, manual or electronic, which provides controlled access to all copies of the vessel's master key. Vessel does not have a master key control system and the Master has access to master keys. - 46 USC 3106(a)(1)

Fire and Abandon ship drills must be held on each vessel normally on alternate weeks. Vessel was not conducting drills on alternating weeks and was conducting drills monthly. -46 CFR 131.530(b) / 46 CFR 131.535

Vessel must undergo an annual inspection within 3 months before or after each anniversary date. Vessel was found to be operating outside of annual window.

Mariners must meet medical and physical standards. Relief captain did not have valid medical certificate.

Vessel must have a valid load line certificate. Watertight doors and hatches going into engine room and engine room escape hatch were not watertight. Bow thruster seal was not made watertight. ABS is not issuing load line certificate until these items are rectified.

Vessel must maintain a copy of VRP on board and be understood by the crew. There was no VRP on board.

Vessel must have adequate emergency lighting fitted along line of escape. Emergency light in escape hatch was inoperable.

If vessel is to be altered or repaired, the plans and information must be submitted to the [name withheld] were three temporary repairs on the grey water discharge line. Repair proposal needs to be provided.

A vessel of less than 100 gross tons must have adequate emergency lighting fitted along the line of escape to the main deck from accommodations and working (machinery) spaces below the main deck. Engine room escape trunk light was extinguished and tunnel walkway was dim requiring a portable light to escape. Vessel to repair and test. 46 CFR 129.440(a)

Cargo tanks in which Grade B or C liquids that are to be transported shall be fitted with either individual pressure-vacuum relief valves which shall extend to a reasonable height above the weather deck. Cargo tank safety relief valves shall be inspected at least once in every 2 years. The safety relief valve discs must be lifted from their seats in the presence of a marine inspector by either liquid, gas, or vapor pressure at least once every 5 years to determine the accuracy of adjustment and, if necessary, must be reset. The relief valves for the methanol tanks did not have any records of being tested. 46 CFR 32.55-20(c), 46 CFR 38.25-10(a), 46 CFR 38.25-10(b)

The following matters must be entered in each vessel's logbook: - Tests and inspection of Steering Gear, Whistle, and Means of Communication. Before departure. - Verification of Compliance with Applicable Stability Requirements. - Tests of Emergency Lighting and Power. Weekly, monthly, and twice-yearly. Complete tasks and log. 46 CFR 131.620(b),46 CFR 131.620(d), 46 CFR 131.620(g),

At least one abandon-ship drill each 3 months must be held at night, unless the master determines it unsafe. Vessel was completing abandon ship drills, but none logged at night. 46 CFR 131.530(b)(9)

Each OSV must carry lifebuoys approved under approval series 160.150 or 160.050. Aft 01 deck lifebuoy grab line was frayed in several locations. Replace or renew grab line. 46 CFR 133.70(a)

Vessel has stress fracture on port bow hull and inset about six inches long - 30"x18" impact area four feet above the water on cord seam. Vessel must comply with ABS rules for construction. Vessel to repair vessel hull to satisfaction of CG and ABS. Vessel not to depart AOR or engage in commercial ops until repair complete. Vessel authorized to transit to shipyard in Orange for repairs. 46 CFR 127.210

The following worklist items were discovered by the inspection team and must be cleared prior to the vessel receiving credit for the drydock/ISE: 1.) #3 Starboard liquid mud tank had a wasted sounding tube in it. Renew as original. 2.) Port grey water tank discharge valve was wasted. Renew as original. 3.) Starboard grey water tank discharge valve was wasted. Renew as original.

Permanently attach the switchboard to the deck.
