PSCO observed leaks of flammable liquids. #2 Aux engines no 3, 4, and 5 cylinders were leaking and the no. 1 aux engine's fuel pump was leaking.

Cargo spaces on cargo ships of 2,000 gross tonnage and upwards shall be protected by a fixed carbon dioxide or inert gas fire-extinguishing system complying with the Fire Safety Systems Code. PSCO observed delivery lines for CO2 to cargo holds No. 2 & 3 were completely broken; therefore one CO2 system and smoke extraction systems were inoperable for those holds. Vessel was unable to provide exemption certificate IAW SOLAS 2014 #12/10.7.1.4.

Such penetrations shall be suitably insulated by extension of the insulation at the same level of one division.
