The fire resistance of doors shall be equivalent to that of the division in which they are fitted. PSCO observed both main galley doors were marked as class "C" division doors when main galley bulkhead is required to be class "A" division per SOLAS.

The purpose of this regulation is prevent ignition of combustible materials or flammable liquids. For this purpose, the following functional requirements shall be met. Means shall be provided to control leaks of flammable liquids. PSCO observed leaks of flammable liquids. #2 Aux engines no 3, 4, and 5 cylinders were leaking and the no. 1 aux engine's fuel pump was leaking.

To assist in casualty investigations, cargo ships when engaged on international voyages, shall be fitted with a VDR. PSCO observed 01 of the VDR microphones connected to the VDR was inoperable and causing an alarm on the bridge. Vessel had flag dispensation, but made no report to port authority as required on dispensation.

Cargo spaces on cargo ships of 2,000 gross tonnage and upwards shall be protected by a fixed carbon dioxide or inert gas fire-extinguishing system complying with the Fire Safety Systems Code. PSCO observed delivery lines for CO2 to cargo holds No. 2 & 3 were completely broken; therefore one CO2 system and smoke extraction systems were inoperable for those holds. Vessel was unable to provide exemption certificate IAW SOLAS 2014 #12/10.7.1.4.

Where "A" Class divisions are penetrated, such penetrations shall be tested in accordance with the Fire Test Procedures Code. Such penetrations shall be suitably insulated by extension of the insulation at the same level of one division. PSCO observed large crack in the putty on in the CO2 room; therefore not maintaining the A-60 integrity.
