The company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations and with any additional requirements which may be established by the company. In meeting these requirements, the company should ensure that: Inspections are held at appropriate intervals; any non conformity is reported, with its possible cause, if known; appropriate corrective action is taken; and records of activities are maintained. Objective evidence discovered during the COC inspection revealed the following non-conformities which contributed to a serious systematic failure and lack of implementation of the SMS policy onboard in the areas of training, familiarization, record keeping, and maintenance. -Vessel's PMS, code 20126, for Fire Fighters Outfit Inspection requires "Check all Fireman's Outfits are in place, properly arranged, and is in serviceable condition." Vessel is required to have four firefighting outfits; three on board were discovered to be no longer serviceable due to delamination of heat radiating and water resistant foil. Firefighting outfit’s were documented as being satisfactory on 07MAR2019. -As per Ship's Cargo Operations Manual 6.4: "It is important that the integrity of the cargo tanks and venting system is maintained to prevent loss of inert gas or ingress of air. Any reported leaks must be repaired at the earliest opportunity..." PSCOs observed a clamp-type patch approximately 3"x3" on the vessels inert gas distribution line. Inert Gas is escaping from the patch and can be heard hissing from the vessel’s raised catwalk and port walkway. The vessel’s [name withheld] reported that the patch had been there for approximately 3-4 months. There is no record of deterioration to the pipe or application of the temporary repair, and there is no documentation attesting to a plan for permanent repair. -Page 41 of the Vessels training manual has a time requirement and training standard of two minutes. During the abandon ship drill it was discovered that crew members had not been adequately trained in the use of the immersion suits on board the vessel. Only two crew members were able to properly don the on board immersion suit and required life jacket. After further training and instruction, crew demonstrated proper donning of immersion suits and life jackets but exceeded training manual time requirement by five minutes. Vessel recorded conducting a successful abandon ship drill on 08APR2019. -Vessel PMS Code 256 “Cargo Line and Manifold Inspection” requires a monthly inspection of the cargo deck equipment to include: “Check the Cargo Line Electrical Bonding for integrity.” Bonding lines on the hazardous area of the deck were discovered to be improperly repaired (1) and detached (1). PMS was completed and the system was documented to be satisfactory 25MAR19.

Where the emergency source of electrical power is a generator, it shall be: Started automatically upon failure of the main source of electrical power supply unless a transitional source of emergency electrical power in accordance with paragraph 3.1.3 is provided. While testing the emergency generator with the Electrician and Chief Engineer, the emergency generator failed after two seconds of operation. Upon further examination of the system and the fuel tank supply line, it was observed that the fuel shut off valve was in the closed position. No crew member knew why or how long the fuel valve had been in the closed position.

For tankers of 20,000 tonnes deadweight and upwards, the protection of the cargo tanks shall be achieved by a fixed inert gas system in accordance with the requirements of the Fire Safety Systems Code, materials used in inert gas systems shall be suitable for their intended purpose. PSCOs observed an unapproved clamp-type patch (3"x3") on the vessel's inert gas system. Inert gas is escaping from the patch and can be heard hissing from the vessel's raised catwalk and port walkway. The vessel's third mate reported that the patch had been there for approximately 3-4 months. No further documentation was available. Neither the vessel's Captain or Chief Mate were aware of the patch.

A cargo tank of the tank vessel connected to a vapor collection system must be equipped with a permanent or portable cargo gauging device that — is a closed type as defined in 46 CFR 151.15.10(c) that does not require opening the tank to the atmosphere during cargo transfer. In the CCR, PSCOs found displays for tank level readings for tanks 3S ,4S, and 7S to be malfunctioning.

The machinery, boilers, and other pressure vessels, associated piping systems and fittings shall be of a design and construction adequate for the service for which they are intended and shall be so installed and protected as to reduce to a minimum any danger to persons on board, due regard being paid to moving parts, hot surfaces and other hazards. PSCO observed water dripping (multiple drops a second) from Main Engine Jacket Preheater and the Fresh Water pump. According to Chief Engineer [name withheld] Preheater gasket and the Fresh Water pump mechanical seal are failing and need to be replaced. No records of the issue were available.
