15108- Objective evidence discovered during an expanded ISM exam revealed the following non-conformities: the vessel failed to fully implement the requirements of the ISM Code through their SMS procedures as evident by the following identified deficiencies indicating that the ship and/or company are not meeting the SMS requirements. Recommend an external audit.

No. 1- The SMS should include procedures ensuring that non-conformities are reported to the company. Vessel's SMS states for Engine Room maintenance, "All crew members shall be responsible for reporting to Master/Chief Engineer any malfunctions or defects discovered and then the company shall be informed." Deficiencies 2-5 were not logged and no submissions to the company could be produced.

No. 2- The Company should establish procedures to ensure that the ship is maintained in conformity with provisions of relevant regulations and ensures inspections are held at appropriate intervals. Vessel's SMS states, "When during routine inspection and certain parameters are found outside of manufacturers recommended limits, the Chief Engineer shall schedule necessary maintenance as soon as practical.” PSCO observed that routine inspections on equipment were being recorded as satisfactory with no additional notes. It was evident during the PSC exam that equipment was operating outside of manufacturers intended limits to include substantial active fuel and lube oil leaks pooled underneath generators and main engine, active hydraulic oil leak from main engine shaft seal, and lagging missing from high temperature equipment actively leaking oil creating an unsafe and flammable condition on the vessel.

Means shall be provided to control leaks of flammable liquids. PSCO observed pools of fuel oil and lube oil throughout the engine room on and beneath equipment. Most notably, the #1 ship service diesel generator and the main engine. The #1 generator had approximately 3 liters of lube oil and oil soaked rags beneath it. The bilge beneath the main engine was saturated with a fuel oil and lube oil mixture.

The machinery, boilers, and other pressure vessels, associated piping systems and fittings shall be of design and construction adequate for the service for which they are intended and shall be installed and protected as to reduce to a minimum any danger to persons on board, due regard being paid to moving parts, hot surfaces and other hazards. PSCO observed missing lagging on the ship service diesel generator’s turbo charger and exhaust connections. This created a hazard to crew members and a significant fire hazard in conjunction with the fuel and lube oil leaks on the generators.

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. The PSCO observed approximately three liters of hydraulic oil in a retro fitted container below the shaft seal. Additionally, the shaft seal hydraulic oil tank was below the required level. These conditions pose a potential flooding hazard and loss of propulsion for the vessel.
