Each towing vessel must be fitted with a general alarm that...has installed, in the engine room and any other area where background noise makes a general alarm hard to hear, a supplemental flashing red light that is identified with a sign that reads: “Attention General Alarm—When Alarm Sounds or Flashes Go to Your Station”. Engine room space does not have a working flashing red light and no alternative way to notify crew in event of an emergency. Vessel is restricted to Lakes, Bays, and Sounds (LBS). In addition to the manning requirement for LBS (5 total crew), one additional crew member shall monitor all personnel in the engine room and act as a notification in the event of an emergency. 46 CFR 143.235(b)(3)

Each remote valve control must be marked in clearly legible letters, at least 25 millimeters (1 inch) high, indicating the purpose of the valve and the way to operate it. Identified missing labeling of the fuel shut off on the starboard side aft location. 46 CFR 143.260(e)

The master or person in charge of a ship...shall ensure that one or more placards meeting the requirements of this section are displayed in prominent locations andin sufficient numbers so that they can be read by the crew...Each placard must be at least 20 cm (8 in) wide by 121/2 cm (5 in) high, made of a durable material, and legible. The following requirements should also be prominently stated:(1) The discharge of all garbage is prohibited into the navigable waters of the United States and into all other waters except as specifically allowed;(2) The discharge of all forms of plastic into all waters is prohibited;(3) A person who violates the above requirements is liable for civil and/or criminal penalties; and (4) Regional, state, and local restrictions on garbage discharges also may apply. No garbage placard was identified meeting these requirements. Install a garbage placard meeting the requirements of 33 CFR 151.59 on board. 33 CFR 151.51(e) 33 CFR 151.59

Machinery...must be designed and maintained to provide for safe operation of the towing vessel and safety of persons onboard under normal and emergency conditions. The port side generator was identified having a leaking gasket at the exhaust manifold and lube oil leak on the governor. 46 CFR 143.205

All fire hoses must be inspected for excessive wear... The port and starboard fire hoses on deck were worn and leaking excessively under water pressure indicating excessive wear and in need of replacement. Witnessed operator replace and test both hoses. Deficiency rectified. 46 CFR 142.240 (a)(7)

Survival craft must have an aggregate capacity to accomodate...100% of the total number individuals onboard. COI states vessel is allowed to have 7 total persons allowed. However, the vessel is currently only outfitted with a 6 man life raft. When operating greater than 1 mile from shore, vessel is restricted to 6 total persons onboard. 46 CFR 141.305(d)

Machinery must be designed and maintained to provide for safe operation under normal and emergency conditions. Plastic bowls on both main engine water separator filters were observed without flame shields. Install flame shields or replace with metallic bowls. 46 CFR 143.205

The vessel owner or operator must...clearly mark all restricted areas and indicate that access to the area is restricted and that unauthorized presence within the area constitutes a breach of security. No security signage was posted. 33 CFR 104.270(c)(6)

Electrical systems and equipment must...minimize system failures and fire and shock hazards. MI witnessed multiple residential style power strips. 46 CFR 143.400(a)

An exposed hazard such as gears and rotating machinery, must be protected by a cover, guard or rail. This is not meant to restrict access to towing equipment such as winches, drums, towing gear or steering compartment equipment necessary for the operation of the vessel. MI observed exposed rotating machinery on both generators. Rectify in the presence of attending MI within 30 days.

Each integral fuel tank must have a vent that connects to the highest point of the tank, discharges on a weather deck through a bend of 180 degrees, and is fitted with a 30-by-30-mesh corrosion-resistant flame screen. MI observed holes in flame screen on 5P fuel tank vent. Rectify in the presence of attending MI within 14 days.

Emergency lighting must be provided for all internal crew working and living areas. Emergency lighting sources must provide for sufficient illumination under emergency conditions to facilitate egress from each space and must be either:(1) Automatic, battery-operated with a duration of no less than 2 hours; MI observed engine room emergency lighting did not illuminate during loss of power. Rectify in the presence of attending MI within 14 days.

All owners and operators of an existing towing vessel must have a valid COI prior to the specified phase-in period. This vessel is one of the associated fleet that has not obtained a valid COI. The vessel shall be made available for a Coast Guard COI inspection within 90 days. If an inspection is not completed prior to this date, the action for this deficiency shall be changed to a 60 (Rectify deficiencies prior to movement) and the vessel may no long be permitted to operate.

The vessel is currently broadcasting (1) Improper MMSI Number,(2) Improper IMO Number,(3) Improper Call Sign,(4) Improper Draft, (5) Improper Length and Beam.
