Each lifebouy must be marked in capital letters with the name of the vessel and have reflective tape. Name of the vessel and reflective tape are missing. Mark name of vessel and install reflective tape on lifebouy.

Machinery and electrical systems must be designed and maintained provide safe operation. Engines are missing guards over belts.

The fire hose must be a lined commercial firehose. The hose onboard is not a lined commercial fire hose. Replace hose with lined commercial fire hose.

(a) Paints, coatings, or other flammable or combustible products onboard a towing vessel must be stored in a designated storage room or cabinet when not in use. (b) If a storage room is provided, it may be any room or compartment that is free of ignition sources. (c) If a dedicated storage cabinet is provided it must be secured to the vessel so that it does not move and must be either: (1) A flammable liquid storage cabinet that satisfies UL 1275 (incorporated by reference, see § 136.112 of this subchapter); or (2) A flammable liquid storage cabinet that satisfies FM Approvals Standard 6050 (incorporated by reference, see § 136.112 of this subchapter); or (3) Another suitable steel container that provides an equivalent level of protection. (d) A 40–B portable fire extinguisher must be located near the storage room or cabinet. This is in addition to the portable fire extinguishers required by tables 142.230(a) and 142.230(b) of this part. Flammable products (solvents) found in machinery space. Remove all flammable products from vessel until approved locker installed. Install locker and required extinguisher.

Fire-detection systems, and fixed fire-extinguishing systems, including ventilation, machinery shutdowns, and fixed fire-extinguishing system pressure-operated dampers on board the vessel, must be inspected or tested at least once every 12 months. Full records of inspection/testing not on board vessel. Provide records.

The master of a towing vessel must ensure that each crewmember participates in fire-fighting drills and receives instruction at least once each month. Fire drills conducted inconsistently (not monthly). Ensure fire drills conducted and recorded monthly.

Portable and semi-portable fire extinguishers must be inspected, maintained, and tested in accordance with the inspection, maintenance procedures, and hydrostatic pressure tests required by Chapters 7 and 8 of NFPA 10. Monthly inspections required by NFPA 10 may be conducted by the owner, operator, person-in-charge, or a designated member of the crew. Crew not performing monthly inspections. Record inspections.

All crewmembers must be provided with health and safety information and training. (1) Content and procedures of the owner or managing operator's health and safety plan; (2) Procedures for reporting unsafe conditions; (3) Proper selection and use of PPE appropriate to the vessel operation; (4) Safe use of equipment including deck machinery, rigging, welding and cutting, hand tools, ladders, and abrasive wheel machinery found onboard the vessel; (5) Hazard communication and cargo knowledge; (6) Safe use and storage of hazardous materials and chemicals; (7) Confined space entry; (8)Click to open paragraph tools Respiratory protection; and (9) Lockout/Tagout procedures. Refresher training must be repeated annually. Vessel had no record of initial or refresher training. Record training.

A towing vessel must carry adequate and up-to-date charts, maps, and nautical publications for the intended voyage. Charts/pubs out of date nor updated. Updated electronic charts onboard. Provide updated nautical publications electronically or in paper form.

The officer in charge of a navigational watch must conduct a navigation assessment for the intended route and operations prior to getting underway. Navigation Assessments not being conducted on short transit evolutions. Ensure NAV assessments conducted each time prior to getting underway.

Fuel systems for towing vessel main engines and generators must have a documented maintenance plan to ensure proper operation of the system. No plan onboard. Document plan.

Individuals, who are not crewmembers, on board a towing vessel must receive a safety orientation prior to getting underway or as soon as practicable thereafter, to include: (a) The location, operation, and use of lifesaving equipment; (b) Emergency procedures; (c) Methods to notify crewmembers in the event of an emergency; and (d) prevention of falls onboard. Provide orientation to non crew.

Towing vsls must be equipped with an illuminated magnetic compass. Compass light inoperable. Make permanent repairs. Vsl restricted to day time ops until corrected.

Each towing vsl must be equipped with nav lights IAW 33 CFR 84. STBRD green nav light did not illuminate. Make permanent repairs. Vsl restricted to daytime / clear wx ops until corrected.

Each towing vessel must have installed, in the engine room and any area where background noise makes a general alarm hard to hear, a supplemental flashing red light for the general alarm. General alarm red light inoperable. Provide proof of operable flashing red light in the engine room for the general alarm.

Each ring lifebuoy (RLB) with a floating water light must be attached by lanyard of at least 3 ft with a quick-release corrosion-resistant clip securing the water light around the body of the RLB. No quick-release clips on RLBs.

An exposed hazard such as gears and rotating machinery, must be protected by a cover, guard, or rail. M/E and steering gear had exposed rotating parts. Provide protection.

Electrical equipment and cables must be in an enclosure and suitably protected from wet and corrosive environments. PT GEN battery cover was missing. Make permanent repairs.

The MAWP must be indicated by a stamp or nameplate visible to the crew on all pressure vsls (PV). PT PV nameplate/placard was painted over. Make visible.

All owners and operators of an existing towing vessel must have onboard a valid COI prior to the specified phase-in period. This vessel has not scheduled an inspection or obtained a valid COI. The vessel shall be made available for a Coast Guard COI inspection immediately and the vessel may no longer be permitted to operate.

The vessel shall be made available for a Coast Guard COI inspection within 90 days. If an inspection is not completed prior to this date, the action for this deficiency shall be changed to a 60 (Rectify deficiencies prior to movement) and the vessel may no long be permitted to operate.
