Each survival craft shall be stowed: in a state of continuous readiness so that two crew members can carry out preparations for embarkation and launching in less than 5 min. While attempting to lower the starboard side lifeboat to the embarkation deck, PSCO observed an excessive amount of pain covering the entire lifeboat davit launching appliance, inhibiting the crew from launching the lifeboat for at least 20 minutes.

The function of fixed fire detection and fire alarm systems shall be periodically tested to the satisfaction of the Administration by means of equipment producing hot air at the appropriate temperature, or other listed means. The Chief Engineer was unable to produce proper testing equipment for the fixed fire detections and fire alarm systems.

In cargo ships: of 1,000 gross tonnage and upwards, the number of fire hoses to be provided shall be one for each 30m length of the ship and one spare, but in no case less than five in all. During a test of the emergency fire pump, the forward fire hose suffered catostrophic failure. Ship does not meet the required number of fire hoses now.

PSCO observed that two self closing fire doors did not self close. One door was on the main stairway on the 2nd deck and the other was leading from the galley to the main passageway.

Every company should develop, implement and maintain a safety management system which includes the following functional requirement: Instructions and procedures to ensure safe operation of ships and protection of the environment in compliance with relevant international and flag and state legislation. In accordance with the Ship's Safety Management System the vessel is required to ensure all manufacturers' maintenance manuals are readily available on Critical Equipment. PSCO requested testing procedures for the fire detection system which is Critical Equipment. The Chief Engineer stated that there was no manual on board. In addition to that, the last three months of Critical Equipment checks were not signed by any of the appointed Officers.
