PSCO observed an increase in the contents of the sludge tank when reviewing the Oil Record Book without a corresponding entry to explain the increase.

In Cargo ships, the failure of such gauges or overfilling of the tank shall not permit release of fuel into the space. The Administration may permit the use of oil-level gauges with flat glasses and self-closing valves between the gauges and fuel tanks. PSCO observed hold back devices on self closing valves above and below sight glasses.

On 14SEP17, it was reported to USCG Sector Houston that there was a shipboard oil spill incident that subsequently led to several gallons of oil on deck and in the waters of the U.S. Reports from the vessel indicated that there was either a mechanical failure of the valve between the port and starboard fuel tanks or that there was a mis-alignment of the valve that led to the pollution incident.
