Immersion suits over 10 years of age shall conduct annual seam test in accordance with MSC/Circ. 1047. Vessel reported leak coming from main engine cyl. #4 FO pump. Reported leak rate approx 1L every 15 minutes. Vessel's equipment shall be in satisfactory condition and fit for service in which it was intended. Prior to departure from anchorage, please provide statement confirming satisfactory repairs.

The safety management system should include procedures ensuring that non-conformities, accidents and hazardous situations are reported to the company, investigated and analyzed with the objective of improving safety and pollution prevention. The company should establish procedures for the implementation of corrective action, including measures to prevent recurrence. Materiel deficiencies rendering ventilation inoperable were noted during an additional external ISM verification in April 2019; however the corrective actions were insufficient to prevent recurrence as evidenced by the following found October 2020: as identified in deficiency #2 below. Onboard records indicate that a non-SMS shipboard form is utilized for monthly inspections, which are not reported to the company in NS5. The form does not list all vent locations and does not outline the procedure for inspection. An additional internal audit is required to determine the continued lack of maintenance to the vents, the results of which to be submitted to ABS and USCG, ABS to verify the adequacy of the proposed corrective action.

In ships having ro-ro spaces, a separation shall be provided between a closed ro-ro space and the adjacent weather deck. The separation shall be such to minimize the passage of dangerous vapours. The fwd mooring deck self closing door is wasted and had a hold back securing it in the open position.

Where a fixed fire-extinguishing system not required by Ch. II-2 of the convention is installed, it shall meet the relevant requirements of the chapter. Inspectors noted that the fire extinguishing system for the ballast water treatment house was not ready for operation due to an opened drainage valve. The vessel must make the system ready for operation.

he Purpose of this regulation is to provide additional safety measures in order to address the fire safety objectives of this chapter for ships fitted with vehicle, special category and ro-ro spaces. Vehicle, special category and ro-ro spaces shall be adequately ventilated. Inspectors found wastage on hub covers and or electrical junction boxes separated from the following locations; B-5S(P), BF-4S(P), DF-36(P), and DF-34R(P).

The company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations. The vessel’s SMS PRO-EO-07 “Planned maintains program” states that the generators are essential machinery and unplanned shutdown or overdue maintenance must be documented by a verbal risk assessment between the company and the crew. No risk assessment was conducted or documented on board the vessel. The No.1 SSDG tripped offline in an unplanned shutdown and loss of propulsion. Records indicate that several maintenance items were past due by 900 to 14000 hours. An ISM SMC external audit is required.

Machinery and associated piping service shall be designed and constructed adequate for the service in which they are intended. A soft patch was identified on the air supply to the MDE, No.5 cylinder fuel injector. Once removed air continuously leaked out. Make permanent repairs to the air line to prevent leakage.

Provided a fully functioning planned maintenance program to meet the requirements for the vessel's minimally attended machinery plant. The vessel had ABS' NS Enterprise however it omitted generator maintenance which was being tracked on a separate spreadsheet. The generator that failed while entering port had 20 overdue items ranging from 60 to 14,988 hours overdue.

Cargo ships of 4000 GT and upwards must have at least two independent fire pumps in addition to the emergency pump and be ready for immediate service. One fire pump was disassembled and the other was cross connected to an auxiliary salt water cooling system, providing emergency cooling.

SOLAS(14) II-2/7.3.2 - Provide records that the fixed fire detection system has been tested by means of equipment producing hot air or smoke.

Within engine room, multiple wires surrounding light fixtures were joined via wire nuts not within junction boxes. Ensure all conductors are properly installed. 74 SOLAS (04) II-1 45.5.6.

The existing PSTP for items 9.4 and 9.5 do not adequately describe each step required to complete the procedure, ie.e the procedures list a step that inadvertantly secures the engine while installing the test equipment. This equipment should be installed prior to starting the engine for the test. Address this issue & provide updated PSTP to USCG MSC or ACS per NVIC 01-13, Ch. 1, Enclosure (2)

The existing PSTP for item 9.15 requires a step that is potentially harmful to sensitive electronic equipment. The Reverse Power Relay Test Procedure brought the cycles down to below 58 Hertz. Address this issue and provide updated PSTP to USCG MSC or ACS per NVIC 01-13, Ch. 1, Enclosure (2)

SOPEP equipment does not have non-sparking shovels. Replace shovels with proper non-sparking equipment.

Lifebuoy self-igniting light did not have the proper batteries, i.e. springs versus studs, to ensure proper electrical connection. Replace all batteries with proper batteries on all affected lights.

Fire screen door to wheelhouse must be self-closing. Ensure A-0 fire screen door to wheelhouse is adjusted to be self-closing.
