The company should establish procedures for the implementation of corrective action, including measures to prevent recurrence. Materiel deficiencies rendering ventilation inoperable were noted during an additional external ISM verification in April 2019; however the corrective actions were insufficient to prevent recurrence as evidenced by the following found October 2020: as identified in deficiency #2 below. Onboard records indicate that a non-SMS shipboard form is utilized for monthly inspections, which are not reported to the company in NS5. The form does not list all vent locations and does not outline the procedure for inspection. An additional internal audit is required to determine the continued lack of maintenance to the vents, the results of which to be submitted to ABS and USCG, ABS to verify the adequacy of the proposed corrective action.

The vessel’s SMS PRO-EO-07 “Planned maintains program” states that the generators are essential machinery and unplanned shutdown or overdue maintenance must be documented by a verbal risk assessment between the company and the crew. No risk assessment was conducted or documented on board the vessel. The No.1 SSDG tripped offline in an unplanned shutdown and loss of propulsion. Records indicate that several maintenance items were past due by 900 to 14000 hours. An ISM SMC external audit is required.
