PSCO observed a failure of the emergency generator to start automatically and take the emergency load required by paragraph 4 of this regulation within 45 seconds.

Fire drills should be planned in such a way that due consideration is given to regular practice. The muster list shall showthe duties assigned to the different members of the crew. The fire drill failed - crew could not access the space, did not set up boundaries, did not check adjacent spaces for personnel, there were people in area of drill working and in the way, there were no 1st responder actions. The second fire drill failed. The 1st responder checked the affected space, but did not check adjacent spaces. The firefighting team while on air checked each spaces as opposed to going to the affected space and fighting the fire.
