Loss of hydraulic fluid from one system shall be capable of being detected and the defective system automatically isolated so that the other actuating system or systems shall remain fully operational. The number one steering gear pump does not automatically stop when low hydraulic fluid oil is detected.

After any survey of the ship under regulations 7, 8, 9, or 10 has been completed, no change shall be made in structural arrangements, machinery, equipment and otheritems covered by the survey without the saction of the administration. The Inert Gas Generator drail line was repaired without approval from the administration.

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons onboard. Marine sanitation device had pinhole leaks allowing sewage to spill out creating a danger to persons onboard.

A fixed fire detection and fire alarm system and a sample extraction smoke detection system required in this regulation and other regulations in this part shall be of an approved type and comply with the fire safety systems code. Fire detection system had a fault which prevented the fire alarm system from functioning.
