Hydraulic power operated steering gear shall be operated with following: a low level alarm for each hydraulic fluid reservoir to give the earliest practicable indication of hydraulic fluid leakage. The number two steering pump low hydraulic fluid level alarm float is inoperable.

The Inert Gas Generator drail line was repaired without approval from the administration.

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons onboard. Marine sanitation device had pinhole leaks allowing sewage to spill out creating a danger to persons onboard.

A fixed fire detection and fire alarm system and a sample extraction smoke detection system required in this regulation and other regulations in this part shall be of an approved type and comply with the fire safety systems code. Fire detection system had a fault which prevented the fire alarm system from functioning.
