During inspection of starboard engine room with engines running and underway, inspector’s 4 gas meter alerted for high levels of H2S and CO2. Determine cause and affect repairs so an underway operational test of starboard engine room can be completed.

At each initial and subsequent inspection for certification the owner or managing operator shall be prepared to test the steering systems of the vessel and make them available for inspection to the extent necessary to determine that they are in suitable condition and fit for the service intended. Starboard lazarette outboard jet is leaking hydraulic oil at a rate of approximately 1 drop per 10 seconds when underway. Leak is located on inboard underside.

Main deck interior overhead has small leaks throughout that are present during rain. Make interior cabin weathertight by next credit drydock.

Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at a constant marine load. Vessels STBD Aft engine raw water intake piping at the strainer had a pin hole leak and was actively leaking water into the engine room.

Individual bilge suction lines must be led to a central control point or manifold and provided with a stop valve at the control point or manifold and a check valve at some accessible point in the bilge line. Vessel doesn’t appear to have check valves for each suction line to each space. Vessel shall either install check valves or provide proof of existing check valves.

At each initial and subsequent inspection for certification of a vessel, the owner shall be prepared to conduct tests and have the vessel ready for inspections of machinery, including the following: operational test of all overboard discharge and intake valves. Raw water valves in both lazarettes were seized in the open position.

Turbocharger for forward starboard engine has a u-bracket where bracket bolt is digging into turbo housing. Bolt needs suitable chafing gear or to be relocated.

The discharge ends of fuel tank vent pipes must be fitted with removable flame screens or flame arrestors. The flame screens must consist of a single screen of corrosion resistant wire of at least 30 x 30 mesh. Port fuel tank vent in understairs locker on aft main deck has a 1” opening at base of gooseneck not covered by a flame screen.

At each inspection for certification all observed unsafe practices, fire hazards, and other hazardous situations must be corrected. Excess engine oil found throughout starboard engine room needs to be removed.

Mariner must provide evidence of meeting the medical and physical standards in the section. Licensed mate was missing Medical Certificate. Provide certificate.

Marine employers shall establish programs for the chemical testing for dangerous drugs on a random basis of crewmembers on inspected vessels. Vessel shall produce MIS report showing last years drug testing of employees with number of positive/negative results.

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date:(2) U.S. Coast Pilot or similar publication; (3) Coast Guard Light List; (4) Tide tables. Provide up-to-date publications.

Floating waterlight must have a lanyard of at least 3 feet in length, but not more than 6 feet, securing the waterlight around the body of the ring life buoy. Stern ring buoy floating waterlight lanyard must be secured around the body of the ring buoy with a lanyard between 3 and 6 feet in length.

An enclosed or partially enclosed space within a vessel must be adequately ventilated in a manner suitable for purpose of the space. Electrically powered fire dampers must be made operational to open automatically.

A vessel on a lakes, bays, and sounds, or rivers route must carry: (2) Three hand orange smoke distress signals. Distress signals were added during inspection.

There are numerous leaks throughout the vessel that allows for the ingress of water.

A vessel must be provided with a satisfactory arrangement for draining any watertight compartment, other than small buoyancy compartments, under all practicable conditions. The fixed bilge pumping system in the port side lazarette was inoperable and could not provide dewatering capabilities from the port lazarette bilge.

On a vessel of not more than 19.8 meters (65 feet) in length, the minimum capacity of the fire pump must be 189 liters (50 gallons) per minute at a pressure of not less than 414 kPa (60 psi) at the pump outlet. The pump outlet must be fitted with a pressure gauge. The pressure gauge on the fire pump was inoperable and could not display the pressure output when in operation.

Boundaries that separate machinery spaces from accommodation spaces, service spaces and control spaces must be lined with noncombustible insulation. The insulation in the starboard engine room space was damaged and needs repaired.

Crew training shall be logged or otherwise documented for review by the Coast Guard upon request. There was no record of drills conducted by the crew on board the vessel.

Marine employers shall establish programs for the chemical testing for dangerous drugs on a random basis of crew members on inspected vessels. There was no document on board the vessel to provide evidence of crew members being enrolled in a chemical testing program for dangerous drugs.

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. The ceiling light fixture in front of the starboard restroom was not in operating condition and was not securely affixed to the ceiling.

A vessel must carry one floating water light, unless it is limited to daytime operation, in which case no floating water light is required. The light on the port side top deck was inoperable.

At each inspection for certification and at every other vessel inspection the bilges and other spaces may be examined to see that there is no excessive accumulation of oil, trash, debris, or other matter that might create a fire hazard, clog bilge pumping systems, or block emergency escapes. In the starboard lazarette, there was an excessive accumulation of hydraulic oil in the bilge aft of the steering pump.

The shutoff valve at the tank must be manually operable from outside the compartment in which the valve is located, preferably from an accessible position on the weather deck. The emergency remote fuel shutoff for the starboard engine was inoperable.

Annual inspection. Your vessel must undergo an annual inspection within the 3 months before or after each anniversary date. Vessel did not undergo an annual inspection within 3 months of anniversary date.

A submersible electric bilge pump may be used as a power bilge pump required by Table 182.520(a) only on a vessel of not more than 19.8 meters (65 feet) in length carrying not more than 49 passengers, other than a ferry, provided that: (1) The pump is listed by an independent laboratory as meeting the requirements in UL 1113. MI unable to verify bilge pump specifications due to faded/missing labeling. Vessel Rep unable to produce documentation proving UL approved pumps. Replace all bilge pumps in watertight compartments with approved pumps. 46 CFR 182.850(e)(1)

An inflatable buoyant apparatus must be serviced at a facility specifically approved by the Commandant no later than the month and year on its servicing sticker. 100-person IBA due for annual servicing on 30MAY2020. Vessel is restricted to carrying no more than 37 persons on board. 46 CFR 185.730(a)(1)

Each disposable hydrostatic release unit must be marked with an expiration date of two years after the date on which the unit is installed. Hydrostatic release unit for EPIRB was not marked with required expiration date. Vessel is restricted to voyages less than 3NM from shore. 46 CFR 185.740(b)

It is mandatory that each 406.0-406.1 MHz EPIRB be registered with NOAA before installation and that information be kept up-to-date. EPIRB registration expired on 30MAY2020. Vessel is restricted to voyages less than 3NM from shore. 46 CFR 184.502 [parent] & 47 CFR 80.1061(f)

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI, except during an emergency. During expanded insepction of recent inboard, starboard Hamilton Jet steering sensor replacement and loss of steering, an unrelated electronic sensor was found taped in place at the jet drive.

Shutoff valves, installed so as to close against the fuel flow, must be fitted in the fuel supply lines, one at the tank connection and one at the engine end of the fuel line. The shutoff valve at the tank must be manually operable from outside the compartment. Stbd engine room remote shut off broke in the open position during closure test and could not shut the fuel valve.

The minimum capacity of the fire pump must be 50 gallons per minute at a pressure of not less than 414 kPa (60 psi) at the pump outlet. The pump outlet must be fitted with a pressure gauge. Testing fire hose indicated only 40-45 PSI at gauge (sat visual pressure/spray pattern).

The design of all exhaust systems must ensure minimum risk of injury where persons or equipment might come into contact with an exhaust pipe. Piping, the temperature of which exceeds 150 °F must be suitably insulated where necessary to prevent injuries. Exhaust laggging missing on several sections of exhaust including at two elbows which had a large "split" in the pre-formed lagging covers, and two flexible metallic exhaust unions (all missing sections from 8-12" length each).

The employer shall provide an Employee Assistance Program (EAP) for all crew members. Inspectors and boat crew could not find a posted hot line telephone number.

The number and identification of the items stowed inside, and their sizes, must be marked in clearly legible letters and numbers on each container for life jackets and immersion suits. Vessel has airline style seating, all rows contain 2-3 adult PFD's, but several are not marked for storage.

Electrical equipment used in a machinery space must be dripproof. Starboard engineroom aft MDE, engine mounted Cat Control Electronic box taped together (not dripproof).

Each electric space heater must be provided with a thermal cutout to prevent overheating. Wheelhouse space heater tested and did not appear to have thermal cutout.

Heating equipment must be suitable for marine use. Equipment designed and installed in accordance with American Boat and Yacht Council (ABYC) A-7, “Boat Heating Systems,” or with National Fire Protection Association (NFPA) 302, “Pleasure and Commercial Motor Craft,” complies with this requirement. Unknown space heater was jury rigged in port lazarette, fed by long extension cord from stbd lazarette plug outlet. Repairs or alterations that affect the safety of the vessel include replacement of electrical wiring, steering, propulsion and power supply systems and must not be made without the approval of the cognizant OCMI.The owner or managing operator shall submit drawings, sketches, or written specifications describing the details of any proposed alterations to the cognizant [name withheld] lazarette had space heater jury rigged in bulkhead, with 50' extension cord to stbd lazarette plug outlet.

Propulsion machinery of an unusual type for small passenger vessels must be given separate consideration and is subject to such requirements as determined necessary by the cognizant OCMI. The requirements for machinery propelled vessels are contained in applicable regulations in subchapter F (Marine Engineering) and subchapter J (Electrical Engineering) of this chapter. Recent log entry showed wing station control failed and needed to be trasnferred back to wheelhouse helm. Provide details on the steering system visible and audible alarm system for this vessel and show proper operation of these systems.

Vessel o/o issued CG-835 September 5, 2019, not to carry passengers due to not having the required annual inspection IAW 46 CFR § 176.500(b)(1). (See Admin Activity 6804581)

On a vessel of not more than 65 feet in length and carrying more than 49 passengers, the minimum capacity of the fire pump must be 50 gallons per minute at a pressure of not less than 60 psi at the pump outlet. The fire pump did not provide adequate pressure, visually, which could not be verified due to an inoperable pressure gauge at the pump outlet. Pressure witnessed was not able to reach all areas of the vessel.

Every vessel subject to Part III of Title II of the Communications Act must have a detailed inspection of the radio installation by an FCC-licensed technician in accordance with 47 CFR 80.59 once every five years. The vessel did not have a Radiotelephony certificate onboard. 47 CFR 80.903

All cable and wire must be installed with metal supports spaced not more than 610 millimeters (24 inches) apart, and in such a manner asto avoid chafing and other damage. The use of plastic tie wraps must be limited to bundling or retention of multiple cableinstallations, and not used as a means of support. Portions of wiring in the engine rooms and forward compartments were supported by plastic zip ties.

The master or person in charge of a ship to which this section applies shall ensure that the ship is not operated unless a garbage management plan meeting paragraph (b) of this section is on the ship and that each person handling garbage follows the plan. Vessel does not have a waste management plan onboard. 33 CFR 151.57
