When a nominated surveyor or recognized organization determines that the condition of the ship or its equipment does not correspond substantially with the particulars of the Certificate or is such that the ship is not fit to proceed to sea without presenting an unreasonable threat of harm to the marine environment, such surveyor or organization shall immediately ensure that corrective action is taken and shall in due course notify the Administration. If such corrective action is not taken the Certificate shall be withdrawn and the Administration shall be notified immediately; and if the ship is in a port of another Party, the appropriate authorities of the port State shall also be notified immediately. When an officer of the Administration, a nominated surveyor or recognized organization has notified the appropriate authorities of the port State, the Government of the port State concerned shall give such officer, surveyor or organization any necessary assistance to carry out their obligations under this regulation. When applicable, the Government of the Port State concerned shall take such steps as will ensure that the ship shall not sail until it can proceed to sea or leave the port for the purpose of proceeding to the nearest appropriate repair yard available without presenting an unreasonable threat of harm to the marine environment. PSCO received report of temporary fix for the stern tube seal leak onboard the vessel. It was reported no visible sheen observed beneath the waterline or on the surface. Vessel is expecting permanent repair at next port.

"...When applicable, the Government of the Port State concerned shall take such steps as will ensure that the ship shall not sail until it can procedd to sea or leave the port for the purpose of proceediing to the nearest appropriate repair yard available without presenting an unreasonable threat of harm to the marine environment." PSCO observed that approximately 114 liters of lubricating oil is unaccounted for from the stern tube seal gravity tanks. The crew suspects a stern tube seal leak. Although no surface sheen is currently visible, there is a high probability of continuous oil discahrge into the surrounding water, posing an imminent risk to the marine environment.

Resolution: Recieved satisfactory class report of temporary repairs to the stern tube. Cleared the 60 to allow the ship to sail but issued a 40 to ensure rectification with permanent repairs.

The main inlets and outlets of all ventilation systems shall be capable of being closed from outside the spaces being ventilated. PSCO observed the ventilation ducts in cargo bays 30 to 38 were unable to be closed due to excessive rust.
