Plans listed in 46 CFR 127.110 must be submitted for approval for dual certification T&L prior to conducting OSV operations in Sector Southeastern New England's zone as defined in 46 CFR Subchapter L. All modifications must be made, inspected and approved by an attending USCG Marine Inspector.

Prior to operations as an Offshore Supply Vessel in Sector New England's zone the vessel must undergo an inspection to verify compliance with 46 CFR Subchapter L, to the satisfaction of the attending USCG Marine Inspector.

Stability information (stability details indicated on the Certificate of Inspection, a stability letter, or a stability booklet), is required on certain vessels by paragraphs (b), (c), or (d) of this section. Enough stability information, including stability calculations and assumptions made to use them, must be provided to allow the master to be able to determine operating guidelines, loading restrictions, and ensure compliance with the applicable intact and damage stability regulations of this chapter. Vessel had six 55GAL drums filled with water and 2 55GAL jugs filled with water adding approximately 4,000lbs of weight. The stability letter only allows 1,000lbs of deck cargo.

When, in accordance with § 178.210 of this chapter, a vessel must be provided with a stability letter, the stability letter must be posted under glass or other suitable transparent material, such that all pages are visible, at the operating station of the vessel. If posting is impracticable, the stability letter must be kept on board in a weathertight container readily available. Stability letter was not posted.

The Certificate of Inspection and any SOLAS Certificates must be posted under glass or other suitable transparent material, such that all pages are visible, in a conspicuous place on the vessel where observation by passengers is likely. If posting is impracticable, such as in an open boat, the certificates must be kept on board in a weathertight container readily available for use by the crew and display to passengers and others on request. COI was not posted.

Each survival craft must be: Stowed so that when the vessel sinks the survival craft floats free and, if inflatable, inflates automatically; Vessels painter line was not attached to the weak-link which would allow for the survival craft to deploy as intended.

Flexible nonmetallic hose may be used in lube oil, fuel oil and fluid power systems, subject to the following requirements: Flexible hose runs must be visible, easily accessible, protected from mechanical damage, and must not penetrate watertight decks or bulkheads. Vessels lube oil return line on both port and stbd MDE was resting on hot surface and needed protection.

Plans listed in 46 CFR 127.110 must be submitted for approval after the owner or builder applies for inspection. Vessel has received approval of all plans required for Subchapter L. All modifications must be made and inspected and approved by an attending Marine Inspector.

A vessel must have stability details indicated on its Certificate of Inspection or a stability letter. Vessel must provide updated Stability Letter upon receipt from Marine Safety Center. Vessel is restricted to the carriage of 26 passengers OR a combination of 20 passengers and deck cargo not to exceed 1000 lbs.

Provide FCC Safety Radio Certificate.

Provide retro reflective tape for life ring.

Prove proper operation of Port & STBD engine shutdowns for fixed fire fighting system.

Program VHF radio with MMSI number and location data.

Conduct underway MOB drill in the presence of USCG marine inspector.

A vessel to which 46 CFR subchapter T applies may not be operated without having on board a valid COI. Conduct drills and verify name change on lifesaving equipment to the satisfaction of the attending USCG Marine Inspector prior to carriage of passengers.

Each item of survival craft equipment must be of good quality and efficient for the purpose it is intended to serve. The floating waterlight for the life float was inoperable upon inspection. Provide proof of satisfactory operation of the floating waterlight.

A vessel must comply with the applicable requirements for any radio installations, including the requirement for a station license issued by the FCC; stations in the maritime service must be licensed by the FCC either individually or by fleet. A FCC Ship Station license was unavailable upon inspection. Provide proof of a FCC Ship Station license for the vessel.

A vessel must comply with the applicable requirements for any radio installations. MMSI was not inputted into vessel's installed VHF radio. Input MMSI into installed VHF radio.

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date charts and nautical publications. Charts, Coast Pilot [name withheld], and tide tables were out-of-date. Provide proof of new materials.

All machinery and equipment must be maintained in such a manner as to afford adequate protection from causing fire, machinery failure, and personnel injury. Sealant was applied to the starboard hydraulic steering ram, and area below the sealant could not be inspected. Remove sealant on the starboard hydraulic steering ram and make component ready for inspection.
