Upon conducting the fire drill on-board, the vessel’s crew demonstrated a lack of familiarity with firefighting equipment and procedures, presenting a potential risk to both crew safety and the vessel. A follow-up fire drill was conducted; however, the crew failed to properly muster and left one crew member unaccounted for during the simulated CO2 discharge in the engine room.

Resolution: Fire Fighting Equipment Plan (Drawing No. 235-555-01) has been approved as per ABS Engineering review letter WO0487640 Dated 30
December 2025. Fire Fighting Equipment Plan has been posted onboard, verified, and considered satisfactory.
The bunker delivery note shall be accompanied by a representative sample of the fuel oil delivered taking into account guidelines developed by the Organization.* The sample is to be sealed and signed by the supplier’s representative and the master or officer in charge of the bunker operation on completion of bunkering operations and retained under the ship’s control until the fuel oil is substantially consumed, but in any case for a period of not less than 12 months from the time of delivery. Vessel was missing two of the last three fuel oil samples to accompany the associated bunker delivery notes and oil record book entry. Additionally, vessel is to obtain samples for the current load of fuel oil upon next bunkering and retain all fuel oil samples onboard in accordance with regulations which will be confirmed at next annual ACP annual oversight inspection by USCG.

The marking must enable passengers and crew to identify the routes of escape and readily identify the escape exits. Vessel had conflicting photoluminescent strip arrows which were not in accordance with the lifesaving plan’s identified escape routes from accommodation spaces.
