Regardless of the option chosen to obtain a COI, upon obtaining a COI each towing vessel must then undergo a drydock and internal structural examination at the following intervals: A vessel that is exposed to salt water not more than 6 months in any 12-month period since the last examination or initial certification must undergo a drydock and internal structural examination at least once every 5 years.

The master of a towing vessel must ensure that each crewmember participates in fire-fighting drills and receives instruction at least once each month. The instruction may coincide with the drills, but is not required to do so. All crewmembers must be familiar with their fire-fighting duties. This vessel has not been utilized in several months. Firefighting drill have not been completed as required and crew was not familiar with their duties. Complete fire drills on this vessel and conduct proper fire drill in the presence of a CG Marine Inspector.

The officer in charge of a navigational watch must conduct a navigation assessment for the intended route and operations prior to getting underway. The navigation assessment must incorporate the requirements of pilothouse resource management of § 140.640, assess operational risks, and anticipate and manage workload demands. No completed navigation assessments or unfilled navigation assessments found on vessel. Provide master with a means to record navigation assessments in accordance with 46 CFR 140.635 and ensure its completion prior to getting underway.

Prior to getting underway for the first time on a particular towing vessel, each crewmember must receive a safety orientation on: (1) His or her duties in an emergency; (2) The location, operation, and use of lifesaving equipment; (3) Prevention of falls overboard; (4) Personal safety measures; (5) The location, operation, and use of Personal Protective Equipment; (6) Emergency egress procedures; (7) The use and operation of watertight and weathertight closures; (8) Responsibilities to provide assistance to individuals that are not crewmembers; (9) How to respond to emergencies relative to the tow; and 10) Awareness of, and expected response to, any other hazards inherent to the operation of the towing vessel which may pose a threat to life, property, or the environment. Newly hired crew members have not completed required training on this vessel. Conduct training and document as required.

Essential systems or equipment must be regularly tested and examined. Tests and examinations must verify that the system or equipment functions as designed. If a component is found unsatisfactory, it must be repaired or replaced. Engine alarm set points are not set to manufacturers requirements and main engine RPM not displaying. Reprogram set points and repair tachometer as required. Tachometer repaired. 30 day Extension granted to allow for technician to adjust/upgrade engineering alarms.

Electrical systems and equipment must function properly and minimize system failures and fire and shock hazards. Engine room and pilot house breaker panels have exposed contacts from missing breakers. Provide means to prevent accidental contact of exposed wires.

Each towing vessel must carry at least one appropriately-sized lifejacket, approved under approval series 160.002, 160.005, 160.055, 160.155, or 160.176, for each person on board. Vessel missing (1) approved life jacket. Provide life jacket as required.

Means must be provided for stopping each fan in a ventilation system serving machinery spaces and for closing, in case of fire, each doorway, ventilator, and annular space around funnels and other openings into such spaces. Windows in engine room are only capable of being closed from inside the engine room. Provide means to secure ventilation from outside the engine room.

The owner, managing operator, or master of each towing vessel must maintain the required navigational-safety equipment in a fully-functioning, operational condition. Starboard running light not working and mast head light is equipped with a household light bulb. Repair both lights as required.

Means must be provided for closing, in case of fire, each machinery space doorway, ventilator, and annular space around funnels. Second deck of engine room has fixed vent with no way to secure. Provide method to close vent.

Electrical systems and equipment must function properly and minimize system failures and fire and shock hazards. Electrical outlet cover on bridge missing and j-box for lights missing. Repair covers as required.

Each towing vessel must have either a self-priming, power-driven, fixed fire pump, a fire main, and hoses and nozzles in accordance with paragraphs (a) through (d) of this section; or a portable pump, and hoses and nozzles, in accordance with paragraphs (e) and (f) of this section. Portable fire pump not self priming. Provide self priming pump as required.

Each lifebuoy on a towing vessel must Be approved under approval series 160.050 or 160.150. Materials - Grab line. The grab line shall be 3/8-inch diameter polyethylene, polypropylene, or other suitable buoyant type synthetic material having a minimum breaking strength of 1,350 pounds. Grab line for all lifebuoys wasted. Repair or replace as required.

Repairs and minor alterations to existing towing vessels must be made in accordance with this part. New installations that are not replacements in kind must comply with the requirements of Subpart C of this part (starting at 46 CFR 143.500), if applicable. Ship service generator replaced with upgraded system. Provide verification of compliance with the construction and arrangement design standards as required.

Stowage positions for life jackets must be marked in block capital letters and numbers with the minimum quantity, identity, and, if sizes other than adult or universal sizes are used on the vessel, the size of the life jackets stowed inside the container. Life jacket storage containers missing number of life jackets throughout the vessel. Add quantity as required.

Each towing vessel must have either a self-priming, power-driven, fixed fire pump, a fire main, and hoses and nozzles in accordance with paragraphs (a) through (d) of this section; or a portable pump, and hoses and nozzles, in accordance with paragraphs (e) and (f) of this section. The portable fire pump must be self-priming and power-driven. Portable fire pump is not self-priming. Make necessary repairs to make pump self priming.

Each window or portlight, and its means of attachment to the hull or the deckhouse, must be capable of withstanding the maximum expected load from wind and waves, due to its location on the vessel and the vessel's authorized route. Aft bridge window cracked. Replace as required.

The original COI must be framed under glass or other transparent material and posted in a conspicuous place onboard the towing vessel. COI was not properly stored. Stored as required.

Excessive amounts of oil and sludge buildup in the engine room bilge. Remove built up sludge and oil from bilges to the satisfaction of the attending CG Marine Inspector.

Remotely operated fuel shutoff valves are not located directly outside of the fuel oil supply tank. Re-locate valves and remote operations cable to the piping directly outside of the fuel oil supply tanks.

No accurate method provided to determine if the portable fire pump meets the minimum capacity of at least 80 gallons per minute and a minimum discharge pressure of not less than 60 pounds per square inch measured at the pump discharge. Provide proof of minimum flow rate and discharge pressure. Provide means or method used to determine flow rate and discharge pressure.

Navigation assessments are vague in nature as written, and do not consider all elements of the pilothouse resource management and must consider changes in conditions, equipment, crew knowledge/qualifications.

Required Chemical Testing Program does not display Employee Assistance Program informational material to include at a minimum: community service hot-line telephone number for crewmember assistance and employer's policy regarding drug and alcohol use in the workplace.

Vessel does not a documented fuel maintenance plan for the main engines and generator to insure a clean fuel supply is provided. Maintenance plan shall include at a minimum: Fuel filter/water separator replacement intervals, where filters are used, and minimum amount of spares are kept on board.

No proof that vessel personnel or company have knowledge or are in compliance with the United States Environmental Protection Agency (EPA) National Pollutant Discharge Elimination System (NPDES) Small Vessel General Permit for Discharges Incidental to the Normal Operation of Vessels Less than 79 Feet (sVGP).
