15105 – Resources and Personnel - Every Company should develop, implement and maintain a safety management system (SMS) which includes the following functional requirements: instructions and procedures to ensure safe operation of ships and protection of the environment in compliance with relevant Int and Flag State legislation. Objective evidence discovered during an expanded ISM exam revealed the following non-conformities: The vessel failed to fully implement the requirements of the ISM code through their SMS procedures as evident by the following identified deficiencies indicating that the ship and/or company are not meeting the SMS requirements. Recommend an external audit. No. 1 – Crew is not familiar with valve alignment procedures for boiler startup. PSCO witnessed crew misalign valves for boiler, resulting in a low water alarm and safety shutdown of the boiler. No. 2 – Crew is not familiar with testing procedures for inert gas system alarms. PSCO witnessed crew close scrubber water discharge valve resulting in a backflow of water that leaked out of the piping from the scrubber to the boiler. No. 3 – Crew is not familiar with proper testing procedures for fixed fire detection systems. PSCO witnessed crew test 02 detectors in the engine room, unknowingly discharging water mist system. Vessel personnel were not familiar with the established procedures for shipboard operations. 07111 - Personal Equipment for Fire Safety - In tankers, 4 fire-fighter’s outfits shall be provided. Three sets of the fire-fighter’s outfits had holed gloves, two had trousers with tears, and one had a frayed damage hood. Only 1 fire-fighter’s outfit is fully operational. - 74 SOLAS (04) II-2/10.2.3 - 30ac

13199 - Other (Machinery) - Machinery boilers and associated piping systems shall be of a design and construction adequate for the service for which they are intended. PSCO observed active steam leak inside purifier room. - 74 SOLAS (04) II-1/26 - 17a

07105 - Fire Doors/Openings in Fire-Resisting Divisions - The purpose of this regulation is to contain a fire in the space of origin. The fire integrity of the divisions shall be maintained at openings. 02 A-class fire doors leading from engine room to steering room are wasted and unable to properly close. 01 A-class fire door leading from main corridor to engine room does not close properly. - 74 SOLAS (04) II-2/9.1.3 - 17ac

07126 - Oil Accumulation in Engine Room - Means shall be provided to control leaks of flammable liquids. PSCO observed fuel/lube oil leaks on #3 and #4 cylinders on the main engine. - 74 SOLAS (04) II-2/4.1.1 - 17ac

13108 - Operation of Machinery - It shall be possible to control the auxiliary machinery, essential for the propulsion and safety of the ship, at or near the machinery concerned. The local control panel for the #2 boiler is inoperational. - 74 SOLAS (04) II-1/31.5.5 - 50a

13199 - Other (Machinery) - Terminations and joints and all conductors shall be so made to retain the original electrical properties of the cable. The grounding wire on the #1 blower is frayed and not connected. - 74 SOLAS (04) II-1/45.5.6 - 10c

The machinery, associated piping systems, and fittings shall be of a design and construction adequate for the service for which they are intended and shall be so installed and protected as to reduce to a minimum any dangers to persons on board. The main engine oil pressure regulating valve was leaking diesel fuel. 74SOLAS (04cons) II-1/36.1

Per Policy Letter CG-CVC 12-04 vessel shall submit a non-availability report IAW MARPOL Annex VI Regulation 18.

Ed. 2014) II-1/44.2 Each machinery generating set arranged automatically started shall be equipped with starting devices approved by the Administration with a stored energy capability of at least three consecutive starts. Emergency genorator only has manual means of start.

0430 - Water Pipes and Tanks - Ships shall maintain adequate water supply having regard for the size of the crew and duration of voyages. Vessel's water maker is degraded and unable to keep up with the daily consumption needs of the vessel. ILO-147 Co. 68, Article 5 / PWSA 33 U.S.C. 1223 (b)

1599 - Other (Navigator) - Bridge windows shall provide a clear view. Bridge window pane is cracked and "spider-webbed". SOLAS 74 (2009 Cons. Ed.) Ch. V/22.9.4

1830 - Cargo Transfer - Machinery shall be maintained and suitable for purpose intended. #1 cargo pump gasket failed causing steam leak. SOLAS 74 (2004 Cons. Ed) Ch. II-1 / Reg. 26.1

1080 - Boiler Alarm - Boiler #2 currently will not continuously operate due to an alarm (boiler common shutdown) which closes the fuel oil delivery valve and shuts down the boiler. Vessel currently has (01) operational boiler for cargo operations and generating inert gas. 74SOLAS (04 cons. ed.) CH II-1 Reg 26.1

Three cargo tanks were above 8 Percent O2 (5 Port-10 Percent O2, 5 Starboard-10.3 Percent O2, 3 Port-8.1 Percent O2). 74/78 SOLAS (AMD) CH II-2 Reg 62.

All 4 vapor lines located at the manigolds are missing the individual studs. Vessel can not load cargo in U.S. waters until fixed. 46 CFR 39-20-1e To the satisfaction of Class.
