Machinery spaces of category A above 500m3 in volume shall, in addition to the fixed fire-extinguishing system required in paragraph 5.1.1, be protected by an approved type of fixed water-based or equivalent local application fire-extinguishing system, based on the guidelines developed by the Organization. While reviewing Engineering Logbook, PSCO identified entry stating vessel's fixed local application fire-extinguishing system was switched to manual mode due to low level alarm in potable water tank during voyage from Belfast, UK to New Orleans. PSCO observed starboard potable tank depleted of source water, rendering the F/F system inadequate for intended purpose.

Resolution: Received, reviewed and accepted class report from [name withheld] stating the deficiency was rectified. All maine engine overdue maintenance of fuel injectors has been completed and all fuel nozzles were renewed.
The master, owner, operator, agent, or person in charge of any vessel equipped with ballast water tanks that operates in the waters of the United States must follow these practices: Remove fouling organisms from the vessel's hull, piping, and tanks on a regular basis and dispose of any removed substances in accordance with local, State and Federal regulations. Upon arriving to vessel, PSCO observed significant outer skit of marine growth on all sides of vessel caused by severe failure of anti-fouling system on hull. Vessel not authorized to conduct cargo operations in U.S. until rectified.
