PSCO observed hydraulic power pack piping and fittings actively leaking accumulating in a fabricated tray underneath.

PSCO observed an unauthorized electrical installation in the form of a surge protector spliced into a light fixture and zip tied to a cable run outside of the CO² room. Additionally, PSCO observed inadequetly insulated electrical connections in the following locations:
- Both FO purifier electrical motors,
- Both LO purifier electrical motors,
- Sewage treatment plant level sensor,
- Boiler burner junction box,
- Light in emergency diesel generator space.
