The machinery, boilers and other pressure vessels, associated piping systems and fittings shall be of a design and construction adequate for the service for which they are intended and shall be so installed and protected as to reduce to a minimum any danger to persons on board, due regard being paid to moving parts, hot surfaces and other hazards. PSCO observed hydraulic power pack piping and fittings actively leaking accumulating in a fabricated tray underneath. 74SOLAS(04) II-1/26.1

All electrical apparatus shall be so constructed and so installed as not to cause injury when handled or touched in the normal manner. PSCO observed an unauthorized electrical installation in the form of a surge protector spliced into a light fixture and zip tied to a cable run outside of the CO² room. Additionally, PSCO observed inadequetly insulated electrical connections in the following locations: - Both FO purifier electrical motors, - Both LO purifier electrical motors, - Sewage treatment plant level sensor, - Boiler burner junction box, - Light in emergency diesel generator space. 74SOLAS(04) II-1/45.1.3
