Two of the ships lifejackets in the wheelhouse did not have the proper attachments.

Vsl had fire extinguisher bracket not designed for fire extinguisher it was supporting, against manufactuer's recommended practice.

Vessel's lanyards were to long and not secured around the body of the buoy. Secure lanyard to body of buoy and resize line to fit regulations.
