Basic principles underlie the regulations in this chapter and are embodied in the regulations, having regard to type of ship and potential fire hazard: separation of accommodation spaces from the remainder of the ship by thermal and structural boundaries. Fire door to the bridge would not automatically close.

An automatic emergency lighting and power system is one in which a reduction in potential from the ship's service power and lighting plant causes the emergency power source to supply power to the emergency loads.

A certificate issued under regulation 12 or 13 shall cease to be valid in any of the following cases: if the relevant survey and inspections are not completed within the periods specified under regulations 7, 8, 9, and 10. MI's were unable to complete MARPOL and SOLAS surveys.

Each vessel that relies on electricity to power the following loads must be arranged so that the loads can be energized from at least two sources of electricity:(i) Any system identified as a vital system in § 128.130(a) of this subchapter.(ii) Interior lights.(iii) Communication systems.(iv) Navigational equipment and lights.(v) Fire-protection equipment. Vessel emergency generator was not functional at the time of inspection.

Machinery driving forced-draft and induced-draft fans, fuel-oil transfer pumps, fuel-oil unit and service pumps, and similar fuel-oil pumps must be fitted with remote controls from a readily accessible position outside the space concerned so that the fans or pumps may be stopped in case of fire in the compartment in which they are located. The controls must be suitably protected against accidental operation and against tampering and must be suitably marked. remote fuel stops were inoperable at the time of the inspection.

Except as provided by § 131.545(e) of this subpart, ach lifesaving appliance and each item of equipment for a lifeboat, liferaft, survival craft, rescue boat, life float, or buoyant apparatus must be in good working order and ready for immediate use before the vessel leaves port and at any time when the vessel is away from port. Rescue boat was inoperable at the time of inspection.

The requirements of this section apply to OSVs between 100 GRT and 500 GRT or less than 6,000 GT ITC. If a generator provides power for any system identified as a vital system 128.130(a) of this subchapter, at least two power-generating sets must be provided. At least one set must be independent of the main propulsion plant. A generator not independent of the main propulsion plant must comply with 111.10-4(d). The vessel's master reported the port ship service generator was not in satisfactory working condition. You must effect repairs to the satisfaction of the Coast Guard marine inspector. Vessel is authorized to continue operations with temporary generator installation then may transit to Port Fourchon to conduct permanent repairs no later than 10JUN25.

An OSV of less than 6000 GT ITC must obtain and maintain the following international certificates as a prerequisite to obtaining a COI: Vessel is prohibited from international voyages as the CSSC and IAPP are currently invalid until repairs are made and satisfactory to the CG marine inspector. 46 CFR 125.105

Whenever damage or deterioration to hull plating or structural members that may affect the seaworthiness of a vessel is discovered or suspected, the cognizant OCMI may conduct an internal structural examination in any affected space including fuel tanks, and may require the vessel to be drydocked or taken out of service to assess the extent of the damage, and to effect permanent repairs. Valve actuators must be capable of closing the valves under all conditions, except during physical interruption of the power system (e.g., cable breakage or tube rupture). Remote operation of PORT fuel shutoff valves inoperable.

Each piping penetration, in each bulkhead required by this subchapter to be watertight, must meet the requirements for materials and pressure design of subchapter F of this chapter. Port sea chest found to be leaking into the port engine room from strainer housing.

Lifebuoys must be marked in capital block letters with the name of the OSV and port. Identified lifebuoys with no stenciling. Fix to the satisfaction of Class.

At least one lifebuoy on each side must be fitted with a buoyant lifeline. Identified lifebuoys with no line attached. Fix to the satisfaction of Class.

Closures for openings above the freeboard deck must be watertight. Gasket on engine room hatch cover was not watertight. Fix to the satisfaction of Class.

Doors fitted in fire boundary bulkheads of machinery spaces shall be self-closing. Door in machinery space was not self-closing. Fix to the satisfaction of Class.

Vessels engaged on voyages in Sea Area 3 are required to be fitted with an INMARSAT ship earth station ; During system test, INMARSAT-C onboard was determined to not be broadcasting properly; troubleshoot system to restore broadcasting capabilities prior to engaging on voyages within Sea Area 3. 74 SOLAS (14) IV/10.1.1

Operating handle for the rescue boat davit winch was broken off and had to be operated with vice grips. Replace rescue boat davit winch handle.

CO2 storage room cable penetration fire stop was mismatched with two seperate materials. Replace fire stop with approved material. 46 CFR 127.225

Lifebuoy retro reflective tape was torn and faded. Replace retro reflective tape.

The training manual, which may comprise several volumes, shall contain instructions and informaiton, in easily understood terms illustrated wherever possible, on the life-saving appliances proved in the ship on the best methods of survival. The vessel did not have training manuals for the crew onboard. 74 SOLAS (14) III/35.3
