In circumstances of exceptional necessity, Administrations, if in their opinion this does not cause danger to persons, property or the environment, may issue a dispensation permitting a specified seafarer to serve in a specified ship for a specified period...for which he does not hold the appropriate certificate, provided that the person to whom the dispensation is issued shall be adequately qualified to fill the vacant post in a safe manner, to the satisfaction of the Administration. Dispensation granted to allow 2AE to fill 1AE billet until 28FEB2026.

Night vision enhancement equipment shall be fitted. The installed HSC approved camera is inoperable. Flag state dispensation is granted due to delay in receiving camera replacement. Vessel is permitted to utilize hand-held night vision devices in lieu of fixed system. Flag dispensation will expire 23JUL2025. 74 SOLAS (24) X/3.1.2 HSC Code 13.10.1

All automatically or remotely controlled or monitored vital systems addressed by 46 CFR part 62 must be subject to tests and inspections to evaluate the operation and reliability of controls, alarms safety features, and interlocks. Test procedures must be submitted to the Coast Guard for approval. Persons designated by the vessel shall conduct all tests and the Periodic Safety tests shall be witnessed by the Coast Guard. Periodic Safety Test Procedure Documents approved by the Coast Guard must be retained aboard the vessel. Coast Guard approved Periodic Safety Test Procedures were not available during the inspection. Provide approved Periodic Safety test procedures that include the equipment requiring testing outlined in 46 CFR 61.40-6 (Periodic Safety Tests). The test procedure details are required to meet 46 CFR 61-40-10 (Test Procedure Details) for the equipment being tested. 46cfr 61.40-1

The main inlets and outlets of all ventilation systems shall be capable of being closed from outside the spaces being ventilated. Fire dampers SD 434 and SD 431 failed to close. 2000 HSC Code Ch. 7.6.1

Oil filtering equipment shall be of a design approved by the the administration and shall be such as will ensure that any oily mixture discharged into the sea after passing through the system has an oil content not exceeding 15 PPM. A T-fitting with a ball valve was installed on the discharge line to overboard between the OWS overboard valve and the skin valve. MARPOL Annex I Reg. 14.6

Each davit and winch must be marked with a plate or label permanently affixed to a conspicuous pace readily accessible for inspection and sufficiently durable to withstand continuous exposure to environmental conditions. Plate or label must contain: Name and address of manufacturer, manufacturers model identification, name of independent laboratory that witnessed prototype or production tests, serial number, U.S.C.G. approval number, month and year of manufacture, safe working load, the word. “SOLAS”. Areas of major and moderate fire hazard should be enclosed by fire-resisting divisions complying with the requirements in the code. B-Class roller doors are inoperable/not fully closing between passenger spaces. SOLAS (2009 CONS) CH.X HSC Code 2000 7.4.2.1

Vessel's Continuous Synopsis Record (CSR) must be updated. Incorrect classification society listed on CSR for Document of Compliance and Safety Management Certificate issuance. SOLAS (2009 CONS) CH. XI-1/5.1.12(.13)

Engine safety devices shall be capable of being tested IAW on-board automation procedures. Vessel's automation procedures shall be revised to incorporate changes made in procedures during periodic testing. SOLAS (2009 CONS) CH.X HSC Code 2000 9.2.2

All exits should be adequately marked. Exit signage should be placed in messdeck areas IWO lockers. SOLAS (2009 CONS) CH.X HSC Code 2000 4.7.10

Make permanent repairs to the port fire pump. A hole was discovered on the discharge outlet piping of the port fire pump. Repairs will be made to the satisfaction of the attending marine inspector.

Identify root cause analysis of failure of the #2 Marine Evacuation System resulting in the rupture of portions if the evacuation slide.

Make permanent repairs to damaged area located on vessels port quarter. Priovide a detailed repair plan for review and approval before making repairs. Repairs are to be made to the satisfaction of the attending marine inspector or RO acting on behalf of USCG.

Verify arrangement of hydro release units on all life rafts.

Three life raft/MES are not onboard for servicing. Vessel is limited to 727 total persons allowed including crew until all MES and liferafts are serviced and on-board.

Rescue Boat is inoperable. Vessel is restricted to 450 passengers until repaired.

Starboard Fast Rescue Boat Davit inoperable. Vessel is restricted to 450 passengers until repaired. HSC 8.10.4

"Q" Board inverter does not operate properly. Repqir to allow transitional power to all emergency circuits. Vessel is permitted to operate without inverter under either of the following options: Option A: Operate the electric plant in a split bus configuration to separate Switchboard A and Switchboard B; ensuring power supply remains available to 5Q via the ABT. Option B: Operate a closed bus configuration with three parallel generators online – taking operational safety precautions. If a black out occurs, an operational check once power is brought back online to verify the loads from 5Q are available shall be conducted. Additionally, a verification checklist and process to confirm power supply checks after a failure shall be submitted electronically to ABS and USCG.

Structural Fire Protection insulation missing from forward mezzanine level structure must be installed.
