The Oil Record Book shall be completed on each occasion...whenever any of the following machinery space operations take place in the ship: collection and disposal of oil residues(sludge). Since February, three distinct discrepancies in the disposal of sludge are recorded in the ORB. Between 22FEB2026 and 05MAR2026, 0.32m^3(84.5 gallons) of sludge is unaccounted. Between 18MAR2026 and 21MAR2026, 0.04m^3(10.5 gallons) of sludge is unaccounted. Between 22MAY2026 and 17JUN2026, 0.037m^3(9.5 gallons)of sludge is unaccounted. In total, 0.397m^3(105 gallons) of sludge is unaccounted. Vessel must provide 30 days worth of ORB entries on the due date of this deficiency. MARPOL I/17.2.3 50c

...Each transfer pipe system, including each metallic hose, must not leak under static liquid pressure at least 1 1/2 times the maximum allowable working pressure. Vessel has not hydrostatically tested their fuel bunker lines since October 2022. A test must be completed prior to bunkering in U.S. waters. 33 CFR 154.170(c)(4) 40bc

Life-saving appliances required by this chapter should comply with the technical specifications of the LSA Code. The normal equipment in every rescue boat shall consist of: a searchlight with a horizontal and vertical sector of at least 6deg and a measured luminous intensity of 2500 cd which can work continuously for not less than 3 hours. Rescue boat does not have a searchlight. CCSSC 4.3.1 LSA 5.1.2.2.11 40bc

The rings, lines, and letters shall be..permanently marked on the sides of the ship to the satisfaction of the administration. Vessel's "F" and "S" marking for the Freshwater and Summer Line are not permanently marked on either side of the vessel. ILLC (PROT 88) Reg 8 40bc

Vessels with ballast water tanks operating in U.S. waters must...remove fouling organisms from the vessel's hull...on a regular basis and dispose of any removed substances. The vessel's hull has significant marine growth on the stern, port quarter, and starboard quarter. 33 CFR 151.2050(F) 40bc

Subject to the provisions of 3.16.1.2 and 3.16.1.3, The fire safety requirements of bulkheads, decks, doors and stairways should, as far as practicable and reasonable in the opinions of the administration, comply with the applicable requirements of the 1974 SOLAS convention for such bulkheads, doors and stairways. In addition to complying with the specific provisions for fire integrity of bulkheads and decks of cargo ships, the minimum fire integrity of bulkheads and decks shall be as prescribed in tabled 9.5 and 9.6. PSCO observed fire boundaries for bridge, galley, accommodation spaces, and engine room do not comply with the provisions of table 9.5 of 1974 SOLAS II-2. Cargo spaces and accommodation spaces and control stations. The arrangement of each ventilation system should be such that fire in one space should not readily spread to other spaces. PSCO observed the ship only has one ventilation system to supply all compartments on board. CCSSC (97) 3.16.6.1 17bc reduced to 50bc

The Administration shall provide the ship with an appropriate document as evidence of compliance of construction and equipment with the requirements of this regulation. Certification for dangerous goods...is not required for those cargoes specified as class 6.2 and 7 and dangerous goods in limited quantities and expected quantities. PSCO observed the vessel's document of Compliance for the Carriage of Dangerous Goods does not accurately reflect the proper loading locations for the dangerous goods. 74SOLAS(20) II-2/19.4 50bc

Bulkheads forming boundaries between cargo spaces and machinery spaces of category A shall be insulated to "A-60" standard. PSCOs ovserved lack of required A-60 insulation in machinery space below stowage of dangerous goods. 74SOLAS(01) II-2/54.28

Life-saving appliances required by this chapter shall comply with the technical specifications of the LSA code and National Regulations as appropriate. Structural members and all blocks, falls, padeyes, links, fastenings and all other fitting used in connection with launching equipment shall be designated with a factor of safety on the basis of the maximum working load assigned and the ultimate strengths of the materials used for construction. A minimum factor of safety of 4.5 shall be applied to all structural members and a minimum factor of safety of 6 shall be applied to falls, suspensions, chains, links, and blocks. PSCO's observed the the rescue boat weighing 700 lbs with the launching apparaus being aed for a S.W.L. of 1.1 tons, this not meeting safety factors required by the CCS and LSA code. CCS Code 4.3.1 LSA Code 6.1.1.6

Every oil tanker of 150 gross tonnage and above and every ship of 400 gross tonnage and above other than an oil tanker shall be provided with an oil record bok part I. The oil record book, whether as a part of the ship's official loogbook or otherwise shall be in form specified in appendix III of this Annex. PSCO's observed weekly soundings were not recorded in the oil record book. MARPOL ANNEX I Reg 17.1

If the Coast Guard determine that the VRP meet an requirement all requirement of this subpart, this Coast Guard will notify the vessel owner or operator with an approval letter. The VRP will be valid for a period of 5 years from the date of approval...PSCO's discovered the vessels VRP was expired.

The owner or Master of every ship to which this code applies shall ensure that: the condition of the ship and its equipment is maintained to conform with the provisions of this code to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship, persons on board, or the environment. Main port side engine experienced loose valve gear in cylinder number 1 while underway. CCSSC 1.11.1

0120 - Load Line - The international Load Line Certificate (1966) shall not be delivered to the ship until the offer or suveryor acting under the provisions of Article 13 of the present convention has certified that the marks are correctly and permanently indicated on the ships sides. Starboard side load line is marked incorrectly and is not in line witht he plimsoll mark. - ICLL 66/68 Annex 1 reg 9

9900- other (non hazardous) - The strength and construction of hull, machinery casing...shall be sufficient to withstand all foreseeable conditions of the intedned service. Port Side shaft seal leaking 1 gallon every 4 minutes. - CCSSC Ch. 2.4.1 Ship to rectify deficiency during dry dock scheduled for January 2019.

The position of the rudder, if power operated, shall be indicated at the navigation bridge. Conning position on the brige has 02 rudder angle indicators that show different readings of up to 6 degrees off from each other.

In ships of 50M in length and over at least two of the lifebuoys provided with self-igniting lights IAW 4.12.2 shall also be provided with self activating smoke signals. Port and Starboard lifebuoys fitted with smoke signals had the self-igniting lights removed.

03 - 0740 - CCSSC CHAP 3.3.10.1 AN EMERGENCY FIRE PUMP SHALL BE POSITIONED OUTSIDE THE ENGINE ROOM AND PERMANENTLY CONNECTED TO THE FIREMAINS. THE USE OF PORTABLE FIRE PUMPS SHALL BE AVOIDED. SHIP DOES NOT HAVE FIXED EMERGENCY FIRE PUMP.
