During a Marine casualty investigation in to a loss of propulsion it was discovered that the vessel cleans fly wheel pick up sensors as a part of maintenance of the ship. Vessel could not provide organizational policy or procedure from the vessels operator that would require the sensors to be cleaned, maintained, or recorded when the crew cleaned the sensors. An external audit is is required within 90 days. SOLAS(20)IX/3 ISM Code 10

Cargo ships shall carry one or more inflatable or rigid liferaft conforming with the requirements of section 4.2 or 4.3 of the code. The liferaft shall be packed in a container that is as far as practicable watertight. All liferafts onboard vessel have damage to the exterior container affecting watertight integrity. SOLAS CH.III, Reg 31.1.1.2 LSA Code 4.2.6.1.3

The company and the ship shall comply wit the requirements of the International Safety Management Code. The company should ensure that; any non-comformity is reported with its possible cause if known; appropriate corrective action is taken; and records of these activities are maintained. Liferaft maintenance procedures are not being conducted in accordance with vessel's maintenance program. Life saving equipment non-conformities are not being reported and not recorded in accordance with vessel's SMS. Recommend external ISM Audit.

Every company and registered owner shall be provided with an identification number which conforms to the IMO unique company and registered owner identification number scheme. Vessel Continuous Synopsis Record listed incorrect owner with non-standard identification number. 74 SOLAS (2014CE) XI-1/3-1.3 Rectify deficiency within 14 days to the satisfaction of the administration and Coast Guard

The fire protection systems shall be kept in good order so as to ensure their required performance if a fire occurs. Fire detection system in cargo holds is inoperable. Vessel has plans to repair system in Savannah.
