Vessel temporarily installed PAMs unit for current voyage and associated Navy contract. PAMs are portable enclosed spaces and their volumes are subject to inclusion in tonnage. Vessel owner / management is responsible for contacting an authorized measurement organization for a remeasurement determination, except where allowed by a updated tonnage certificate.

Power operated valves installed to comply with the requirements of this section must meet the following requirements: Valve actuators must be capable of closing the valves under all conditions, except during physical interruption of the power system (for example, from cable breakage or tube rupture). The pneumatic valves for 2 port and 2 stbd storage tanks did not close when actuated. Repair at next drydock.

Particular note must be made of circuits added after the original issuance of the COI. The Commanding Officer, Marine Safety Center (MSC), may approve any arrangement, fitting, appliance, or equipment. During inspection, a deck davit with power supply and crane motor control box was noted added to stbd cargo deck. Cables terminated in a plastic box. Unprotected cable runs over 24" were electrical taped to supports and exposed to elements. Cable runs were only partially enclosed in conduit (with the exposed conduit pointing up which could gather water from elements, and making cables susceptible to physical damage from cargo or personnel). Please provide plans to MSC within 30 days. (email: [email withheld]). Necessary corrective work can be completed by end of dry dock (January 31, 2027) to satisfaction of class society-vessel has machinery notation-and USCG.

At each inspection for certification, periodic inspection, and at other times necessary, the inspector will determine that all fire-extinguishing equipment is in suitable condition and may require any tests necessary to determine the condition of the equipment. The generator room's fixed fire-extinguishing 30 second time delay activated at 45 seconds when tested. Demonstrate the proper operation of the generator room's fixed fire-extinguishing time delay in accordance with the system's manufacturer's instructions to the satisfaction of the attending USCG Marine Inspector. 46 CFR 91.25.20 (a)

Periodic safety tests must demonstrate the proper operation of the primary and alternate controls, alarms, power sources, transfer override arrangements, interlocks, and safety controls. Systems addressed must include fire detection and extinguishing, flooding safety, propulsion, maneuvering, electrical power generation and distribution, and emergency internal communications. Test must be conducted at periodic intervals specified by the Coast Guard to confirm that vital systems and safety features continue to operate in a safe, reliable manner. The vessel's crew did not have access the vessel periodic safety test procedures (PSTPs) and therefore could not demonstrate the PSTPs. Demonstrate the vessel's USCG approved PSTPs to the satisfaction of the attending USCG Marine Inspector. 46 CFR 61.40-6

At least two lifebuoys, each with attached self-activating smoke signals, must be stowed where they can be quickly released from the navigating bridge and should, when released, fall directly into the water without striking any part of the vessel. The vessel's lifebuoys with attached self-activating smokes signals are not stowed to fall directly into the water when released. Stow the lifebuoys with attached self-activating smoke signals to ensure they fall directly into the water, when released, to the satisfaction of the attending USCG Marine Inspector. 46 CFR 199.70 (a) (1) (v)

Each immersion suit must be marked in such a way as to identify the person or vessel to which it belongs. One immersion suit is marked with a different vessel's name. Mark the immersion suit with the vessel's name. 46 CFR 199.70 (c) (3)

Each oil tanker of 150 gross tons and above, ship of 400 gross tons and above other than an oil tanker, and manned fixed or floating drilling rig or other platform shall maintain an Oil Record Book Part I. The required entries to the Oil Record Book as detailed in the list of items to be recorded "Part I- Machinery Space Operations" were not being recorded. Enter the required entries into the Oil Record Book Part I from the date of this deficiency. 33 CFR 151.25 (a)

Prove proper operation of vessel's OWS. Demonstrate ability of OCM to alarm and change flow of water.

Provide approval for vessel's PSTP's. Update as needed for new generators.

Submit weight changes to MSC for new generators.

Provide engine room electrical panels P-1 and P-2 with proper closing latches.

Conduct machinery alarms/shutdowns per PSTP's on all main engines and generators. 21MAY2024: Conducted PSTP on generators. Shutdowns SAT. Alarms were inop. CO2 Shutdowns INOP. Follow on inspection required. Sent email to POC for clarification on PSTP submitted.

Makes repairs to broken electrical emergency light in the truster compartment.

Every vessel shall carry at least two firemen's outfits. Vessel is not equipped with firemen's outfits. Equip vessel with a minimum of two firemen's outfits, with all equipment required by 46 CFR 96.35. Vessel is restricted from conducting international voyages until deficiency is rectified.

OWS inoperable-vessel required to retain all bilge/dirty oil water onboard and may not discharge until proved operation of OWS. IOPP void if not repaired within 30 days.

Freezer boxes on main deck installed with extension cords, must provide long electrical cords or install outlets closer to the boxes.

Must install 30x30 mesh flame screen on vent pipe for #3 fuel tank port side.

Must repair or replace explosion proof light cover over port side generator.
