Electrical installations shall be such that the safety of passengers, crew and ship from electrical hazards will be ensured. Found numerous ground wires missing or broken on stbd side lighting fixtures and on purifier room exhaust fan junction box. 74 SOLAS (04) II-1/40.1.3

The means for securing doors weathertight shall consist of gaskets and clamping devices, arranged to be operated from both sides. The weather deck door for the hospital space is missing a lower internal clamping device. 66 ICLL 98 Protocol Annex I Ch II Reg 12

The Company should ensure that each ship is: manned with qualified, certificated and medically fit seafarers in accordance with national and international requirements. One mariner’s TWIC expired in October. Document ID: 06-01-900 indicates the crewing agent is responsible to ensure documents will not expire during shipboard assignment. Any exceptions are to be noted in the Marine Personnel System. Reviewed Marine Personnel System for the mariner and it indicated the TWIC expired in Mar2013. No exceptions were noted in the system. One mariner had a medical waiver, which is required to be kept on board with MMD. Reviewed Document ID 06-01-914 and there is no pre-screening requirement to notify the company if the mariner has a medical waiver (including corrective lenses) prior to assignment on board a vessel. The Administration shall take appropriate steps to ensure uniformity in the implementation and application of the provision of this part in respect of electrical installations. The control system should be designed to include an uninterruptible power supply (UPS) that has sufficient capacity to maintain power to the control system for a period sufficient to bring the emergency power generator online, or if necessary, to safely shut down the equipment. Each vessel must carry a certificate with a valid endorsement while subject to the Bridge-to-Bridge Act. Vessel's Bridge-to-Bridge certificate expired July 16, 2022. Vessel is scheduled to have technician on board at next port in Jacksonville, Florida.

All electrical cables and wiring external to equipment shall be at least a flame-retardant type and shall be so installed as not to impair their original flame-retarding properties. Vessel is missing structural fire protection in 220v junction box. Penetration seals shows signs of melting. Vessel to repair at drydock in September.

The machinery and fittings shall be of a design and construction adequate for the service for which they are intended and shall be so installed and protected to reduce to a minimum any danger to persons on board. Vessel's M/E air receiver safety valve was incorrectly installed.

After any survey of the ship under regulations 7, 8, 9, or 10 has been completed, no change shall be made in the structural arrangements, machinery, equipment and other items covered by the survey, without the sanction of the Administration. Vessel had attached a hose to the shaft seal oil head tank. At the accumulator tank the sight glass packing was leaking and air was leaking from the packing.

Except as provided in paragraphs 5 and 6, life-saving appliances and arrangements required by this chapter shall be approved by the Administration. Various life buoys and areas on rescue boat has unapproved retro reflective tape. Vessel to replace at drydock.

In every ship means shall be provided to prevent overpressure in any part of compressed air systems. Suitable pressure relief arrangements shall be provided for all systems. Vessel had installed a gag on the ship service air receiver.

The fixed fire detection system shall be so designed as to detect rapidly the onset of fire in any part of those spaces. The fire detection system, Loop 11/Deck 5, is in fault.

While getting underway from the pier, the main engine suffered an over speed trip while in astern propulsion. SHIPS SHALL BE MAINTAINED IN COMPLIANCE W/MECHANICAL AND ELECTRICAL REQUIREMENTS OF A CLASSIFICATION SOCIETY RECOGNIZED BY THE ADMINISTRATION; VESSEL'S #3 SSDG UNEXPECTADLY SECURED DUE TO A CRANKCASE OIL MIST ALARM.

The safety management system should include procedures ensuring that non-conformities, accidents and hazardous situations are reported to the company, investigated and analyzed with the objective of improving safety and pollution prevention. The company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations and with any additional requirements which may be established by the company. Objective Evidence: Shipboard maintenance system (Ship Manager) has numerous outstanding items to include 140+ engine room maintenance items overdue, bow thruster inoperable for six months, four thousand hours beyond recommended servicing of main engine suction valves, main engine #4 and #6 cylinder liner scuffing beyond manufacturer specification, deck 5 loop 11 fire detection system inoperable and not reported to class, multiple fire dampers for both cargo and house ventilation (deck workshop, CO2 room, #4 engine room) in need of maintenance (repair or replacement), vessel submitted a life raft servicing extension request to life raft manufacturer but failed to request an extension from flag state authority. An additional external verification is required to determine the perpetuated lack of maintenance, the results of which to be submitted to the USCG.

At all times when the ship is in service, the fire protection systems and firefighting systems and appliances shall be maintained ready for use. Deck 05 fire loop 11 is disarmed due to trouble fault. Demonstrate proper function of Deck 05 fire detection.

Muster and embarkation stations shall be adequately illuminated by lighting supplied from the emergency source of electrical power required by regulation II-1/42 or II-1/43, as appropriate. Free-fall lifeboat muster station lighting not connected to emergency power. Demonstrate proper operation of emergency lighting at muster station.

At all times when the ship is in service, the fire protection systems and firefighting systems and appliances shall be maintained ready for use. Limit switch to initiate time delay for purifier room pilot valve is inoperable. Prove proper operation of limit switch.

Ships, other than passenger ships, of 3,000 gross tonnage and upwards constructed on or after 1 July 2002 when engaged on international voyages, subject to the provisions of regulation 1.4, shall be fitted with a voyage data recorder (VDR). VDR has two error codes. Prove proper function of VDR.

Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. Free-fall lifeboat engine took 1 hour to start due to faulty #2 battery. Demonstrate proper starting of free-fall lifeboat engine. Corrected on the spot.

Means shall be provided whereby normal operation of propulsion machinery can be sustained or restored even though one of the essential auxiliaries becomes inoperative. Vessel was unable to maintain dead slow propulsion due to fuel viscosity.

An alarm sytem shall be provided indicating any fault requiring attention and shall: (51.1.3) activate an audible and visual alarm on the navigation bridge for any situation which requires action by or attention of the officer on watch. The audible alarm for the "sudden stop" on the navigational bridge EOT was inoperable. The light was functioning properly.

99101:All vessels greater than 300 gross tons on a voyage of more than 96 hours is required to submit a Notice of Arrival before departure but at least 96 hours before arriving at the port or place of destination.The vessel departed Beaumont, Texas on December 5, 2019 then submitted a Notice of Arrival on December 7, 2019 at 18:29 EST, with an arrival time to Baltimore, MD on December 10, 2019 at approximately 12:00. 33 CFR 160.212(a)(4)

Steering gear compartment to ensure working access to machinery and controls, to include gratings or other non-slip surfaces to ensure suitable conditions in the event of hydraulic fluid leak (74 SOLAS (14 cons.) II-1/29.13.2). Found excessive hydraulic oil on deck in the steering gear room, creating an unsafe condition for local control for emergency steering.

No change shall be made in the structure, material, or scantlings covered by the survey. Crack found at the elevator door structure, at the engine room deck. Various cracks found in accomadation space doorways, both port/stbd sides.

Cargo ships shall carry a rescue boat complying with section 5.1 of the Code. Grab lines were found to be deteriorated from sun exposure. Navigation light was missing.

Electrical equipment which may constitute a source of ignition shall not be in these compartments. Batteries were found in the EDG space which were not stored properly and had exceeded their service life.

Electrical equipment/wiring shall be approved for explosive petrol and air mixtures. The wiring for multiple cargo space ventilation position switches was exposed and not properly routed through stuffing tubes/J-boxes.

Main switchboard shall be arranged to give easy access. Excess equipment was stored behind the main switchboard, impeding access.

Certificate shall be issued after renewal survey which complies with chapters II-1. the Cargo Ship Safety Construction Certificate had the wrong keel laid date on it.

Such certificate shall be issued by the Administration or by any persons authorized by it. The Administration assumes full responsibility for the certificate. The IOPP had the wrong keel laid date on it.
