The construction of doors and door frames in "A" class divisions, with the means of securing them when closed, shall provide resistance to fire as well as to the passage of smoke and flame equivelant to that of the bulkheads in which the doors are situated. FD TW 2.3 (A-60 fire door) located in the forward sewage room not able to close and latch, compromising the structural fire integrity of the boundary.

The B-15 door located on Deck 03, adjacent to cabin 578, B-15 leading into vitality spa, and B-0 door on deck 14 leading into adventure ocean theatre were unable to close and latch, compromising the structural fire integrity of the boundary.

One manually operated call point shall be loacted at each exit. manually operated call points shall be readily accessible in the corridors of each deck such that no part of the corridor is more than 20 m from a manually operated call point. PSCO observed manual call point D1353 on deck six inoperable when tested. Ships force tested and found call point was damaged and replaced with spare and tested satisfactory and is operating as designed . Action code 10c was noted on form B.
