Doors and door frames in "B" class divisions and means of securing them shall provide a method of closure which shall have the resistance to fire equivelant to that of the divisions. The B-15 door located on Deck 03, adjacent to cabin 578, B-15 leading into vitality spa, and B-0 door on deck 14 leading into adventure ocean theatre were unable to close and latch, compromising the structural fire integrity of the boundary. 74 SOLAS 20 II-2/9.4.1.2.1 15ac

Fire doors in main vertical zone bulkhead, galley boundaries and stairway enclosures, the approximate time of closure for hinged fire doors shall be no more than 40 s and no less than 10 s from the beginning of their movement, FD 3.4.22 (forming MVZ 4) had a closure time of 04 seconds. 74 SOLAS 20 II-2/9.4.1.1.5.2 15ac

Companies, masters and crew members each have responsibiltiies to insure that each crew memeber can make a knowledgeable and informed contribution to the safe operation of the ship. Procedures for the safe operation of the ship include safety, environmental protection, security, and emergency procedures. Three different crew members were unfamiliar with ships procedures regarding passengers in restricted areas. STCW 2017 A-1/14.1 16ac

Paint lockers shall be protected by either a carbon dioxide system, a dry powder system, a water-spraying or sprinkler system. In all cases, the system shall be operable from outside the protected space. PSCO observed that the installed high fog sprinkler system protecting the paint locker is not able to be operated from outside the space. 74SOLAS(20) II-2/10.6.3.1 Code 50c

PSCO observed main vertical zone fire doors obstructed by trash cans on deck 3 and luggage on deck 2. 74SOLAS(20) II-2/9.4.1.1.5.2 Code 10c

Cylinders must always be secured and, when not in use, they must be stowed in a rack in an upright position, with the valve protection cap in place. PSCO observed that 01 oxygen cylinder in the medical oxygen locker in not secured in a rack. 46 CFR 147.60(b)(1) Code 10c

Above the bulkhead deck there shall be at least two means of escape from each main vertical zone or similarly restricted space or group of spaces... PSCO's observed an escape path located outside of the security office's door in way of the escape route that needed a key card to open. 74SOLAS20 II-2/13.3.2.2

Oil fuel pipes which, if damaged, would allow oil to escape from a storage, settling, or daily service tank having a capacity of 500L and above the double bottom, shall be fitted with a cock or valve directly on the tank capable of being closed from a safe position outside of the space. PSCOs observed the starboard side EDG fuel tank quick closing valve required more than one pull to activate. 74SOLAS20 II-2/4.2.2.3.4

Where "A" class divisions are penetrated, such penetrations shall be tested in accordance with the fire test procedures code. Such penetrations shall be suitably insulated by extension of the insulation at the same level of the division. Multiple locations lacking proper insulation. The means of escape, including stairways and exits, shall be marked by lighting or photoluminescent strip indicators placed not more than 300mm above the deck at all points of the escape route, including angles and intersections. Manually operated call points complying wuth the Fire Safety Systens Code shall be installed throughout the accommodation spaces, service spaces and control stations. One manually operated call point shall be loacted at each exit. manually operated call points shall be readily accessible in the corridors of each deck such that no part of the corridor is more than 20 m from a manually operated call point. PSCO observed manual call point D1353 on deck six inoperable when tested. Ships force tested and found call point was damaged and replaced with spare and tested satisfactory and is operating as designed . Action code 10c was noted on form B. Deficiency Code 07106 74SOLAS 20 II-2/7.7

07116- Ventilation - Exhaust ducts from laundries...shall be fitted with filters readily removable for cleaning purposes and suitably located hatches for inspection and cleaning. PSCOs observed missing filters in crew laundry deck 11 and aft mooring deck laundry and latching mechanism was broken on the hatch in crew laundry deck 11. 74 SOLAS 20 II-2/10.7.7

07 - Fire Safety - Storage of combustibles is prohibited in auxiliary machinery spaces having little or no fire. PSCOs witnessed storage of combustible materials in AC room 5-1. 74 SOLAS 09 II-2/9.2.2.3.2(10)

0750-Fire Prevention- Doors in "A" class divisions shall provide resistance to fire as well as to passage of smoke and flame equivalent to bulkheads. Fire screen doors numbered 11.7.4, 8.5.5, 8.5.3, and 6.6.2 could not fully close when tested. 74 SOLAS (14 Cons) II-2/ Reg 9.4.1.1.2

The condition of the ship and it's equipment shall be maintained to ensure that the ship in all respects will remain fit to proceed to sea. After any survey of the ship, no change shall be made in machinery without the sanction of the administration. Vessel recieved casualty of #6 Engine.
