No extensive alterations involving the safety of a tank vessel either in regard to hull or machinery shall be made without the approval of the Commandant. Before such alterations are carried out, copies of plans and specifications in triplicate for the work involved shall be forwarded to the Officer in Charge, Marine Inspection, in whose zone the repairs will be made. Vessel reported a 1.5 inch hole in the #2 void. Hole is located near the bilge knuck and 3' draft section of the void, below the waterline. A dewatering pump if needed is required on board during entirety of transit. Vessel will transit empty of cargo in accordance with approved transit plan to Sterling Shipyard in Port Arthur for repairs to the satisfaction of the attending Marine Inspector.
Action required: 16 - Rectify deficiencies w/in 14 days
