74 SOLAS (2020 Cons.) IX/3.1 ISM Code 10.2.3 The company and the ship shall comply with the requirements of the International Safety Management Code. The company should establish prodecures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations. In meeting these requirements, the company should ensure that appropriate corrective action is taken. The technical and operational nature of deficiencies 02-05 provide objective evidence of a serious failure of the implementation of the ISM Code. A targeted external audit of the ship's maintenance, the condition, company responsibilities satisfactory to the OCMI must be submitted prior to clearance of the deficiency.

74 SOLAS (2004 Cons.) II/10.1 The purpose of this regulation is to suppress and and swiftly extinguish a fire in the space of origin. For this purpose, fire extinguishing appliances shall be readily available. The emergency fire pump non-return valve on the discharge side was replaced with a manual valve rendering the pump unable to be remorely activated as designed.

I/11(a) The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure the ship in all respects will remain fit to proceed to sea without danger to the ship or persons. #2 boiler has multiple cracked pipes internally, rendering the boiler inoperable.

74 SOLAS (2020 Cons.) Ch. II-1/33.1 Every steam pipe shall be so designed, constructed, and installed as to withstand the maximum working stresses to which it may be subjected. PSCOs observed a fractured steam pipe coming from steam injectors on the deck above the cargo pumps in the engine room.

The Company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations and with any additional requirements which may be established by the Company. In meeting these requirements the Company should ensure that: .2 any non-conformity is reported with its possible cause, if known; .3 appropriate corrective action is taken. The technical and/or operational nature of deficiencies 02 - 08 provide objective evidence of a serious failure of the implementation of the ISM Code. The crew is reporting conditions to the company, however the below conditions are not being reported to class/flag nor are they being rectified. An external audit report satisfactory to the OCMI must be submitted prior to clearance of this deficiency. 74 SOLAS (20)IX/3.1 ISM Code 10.2

Main and auxiliary machinery essential for the propulsion and safety of the ship shall be provided with effective means for its operation and control. PSCO observed the main engine is inoperable from the bridge and engine control room. The crew states they have been operating the main engine from the local (emergency) platform since June. This condition has been reported to company without rectification. 74 SOLAS (04)II-1/31.1

The purpose of this regulation is to prevent the ignition of combustible materials or flammable liquids. For this purpose, the following functional requirements shall be met: .1 means shall be provided to control leaks of flammable liquids; PSCO observed oil leaks and accumulated oil: -around the auxiliary blower, all five fuel pumps, all five cylinder heads, crankcase explosion covers 3 - 5, and in the bilge forward of the main engine. -Around the flywheel for generator 1 and 3. Generator 3 had a fabricated catch collecting oil leaks. -Around the fuel tank for the emergency generator. -And near the steering gear pedestal. 74 SOLAS (04)II-2/4.1

NO ELECTRICAL EQUIPMENT SHALL BE INSTALLED IN ANY SPACE WHERE FLAMMABLE MIXTURES ARE LIABLE TO COLLECT INCLUDING THOSE ON BOARD TANKERS OR IN COMPARTMENTS ASSIGNED PRINCIPALLY TO ACCUMULATOR BATTERIES IN PAINT LOCKERS, ACETYLENE STORES OR SIMILIAR SPACES, UNLESS THE ADMINISTRATION IS SATISFIED THAT SUCH EQUIPMENT IS 0F A TYPE WHICH WILL NOT IGNITE THE MIXTURE CONCERNED. PSCO OBSERVED AN EXPLOSION PROOF LIGHT LOCATED IN THE BATTERY ROOM ON THE NAVIGATION BRIDGE MISSING A GLAND IN ONE OF THE STUFFING TUBES. PSCO OBSERVED 2 EXPLOSION PROOF LIGHTS LOCATED IN THE CARGO DECK STORE, WHICH IS INDICATED AS A HAZARDOUS LOCATION PER THE VESSELS APPROVED HAZARDOUS LOCATION PLAN, RETRO-FITTED WITH IMPROPER GLANDS. 74 SOLAS (2004) II-1/45.10.2

THE MACHINERY, BOILERS, AND OTHER PRESSURE VESSELS, ASSOCIATED PIPING SYSTEMS AND FITTINGS SHALL BE OF A DESIGN AND CONSTRUCTION ADEQUATE FOR THE SERVICE FOR WHICH THEY ARE INTENDED AND SHALL BE SO INSTALLED AND PROTECTED AS TO REDUCE TO A MINIMUM ANY DANGER TO PERSONS ON BOARD, DUE REGARD BEING PAID TO MOVING PARTS, HOT SURFACES, AND OTHER HAZARDS. PSCO OBSERVED STEAM CREAKS ON THE STEAM PIPING FORWARD OF COT 6S, FORWARD OF THE COT 3 P/S, ON THE STEAM TURBINES FOR ALL 3 CARGO PUMPS, AND ON THE STEAM RETURN LINE IN THE ENGINE ROOM. ADDITIONALLY, PSCO OBSERVED A FABRICATED CATCH COLLECTING LEAKING JACKET WATER ON GENERATOR 1 AND A FUNNEL DIRECTING JACKET WATER FROM A LEAKING VALVE NEAR THE JACKET WATER COOLING HEAT EXCHANGERS INTO THE BILGE. 74 SOLAS (2004) II-1/26.1

PSCO observed auxiliary boiler #1 inoperable, fixed gas detection for the ballast tanks is inoperable, and fixed gas for the cargo pump room has faults on sampling point #20 due to a bad sensor. 74SOLAS (2020) I/11

THE FIRE RESISTANCE OF DOORS SHALL BE EQUIVALENT TO THAT OF THE DIVISION IN WHICH THEY ARE FITTED, THIS BEING DETERMINED IN ACCORDANCE WITH THE FIRE TEST PROCEDURES CODE. DOORS AND DOOR FRAMES IN "A" CLASS DIVISIONS SHALL BE CONSTRUCTED OF STEEL. DOORS FITTED IN BOUNDARY BULKHEADS OF MACHINERY SPACES OF CATEGORY A SHALL BE REASONABLY GAS TIGHT AND SELF-CLOSING. PSCO OBSERVED CLASS A SELF-CLOSING FIRE DOOR FOR THE BRIDGE STORE LOCATED IN THE MAIN LADDER WELL, CARGO CONTROL ROOM, AND PURIFIER ROOM UNABLE TO SELF-CLOSE. ALL DOORS ARE LABELED AS CLASS A SELF-CLOSING DOORS ON THE APPROVED FIRE CONTROL PLAN. 74SOLAS(2004) II-2/9.4.2.1

FOR TANKERS OF 20,000 TONNES DEADWEIGHT AND UPWARDS, THE PROTECTION OF THE CARGO TANKS SHALL BE ACHIEVED BY A FIXED INERT GAS SYSTEM IN ACCORDANCE WITH THE REQUIREMENTS OF THE FIRE SAFETY CODE. THE INERT GAS SYSTEM REFERRED TO IN CHAPTER 11-2 OF CONVENTION SHALL BE DESIGNED, CONSTRUCTED, AND TESTED TO THE SATISFACTION OF THE ADMINISTRATION. PSCO OBSERVED WATER BACKFEEDING INTO 1G BLOWER #1 LEAKING OUT OF THE BLOWER CASING. ADDITIONALLY, PSCO OBSERVED TEMPORY REPAIR ON SCRUBBER THAT WAS ACTIVELY LEAKING. 74 SOLAS (2004) II-2/5.5.1.1 2007 FSS 15.2.2.1.1

MAIN AND AUXILLARY MACHINERY ESSENTIAL FOR THE PROPULSION AND SAFETY OF THE SHIP SHALL BE PROVIDED WITH EFFECTIVE MEANS FOR ITS OPERATION AND CONTROL. PSCO FOUND MAIN ENGINE TO BE PARTIALLY REPAIRED BY KONGSBERG TECHNICIAN. MAIN ENGINE UNABLE TO BE CONTROLLED FROM BRIDGE CONTROL STATION. PERMANENT REPAIRS TO BE COMPLETED BY 10DEC2024. 74 SOLAS (04) II-1/31.1 Code : 705C

The inert gas system shall be capable of inerting, purging and gas-freeing empty tanks and maintaining the atmosphere in cargo tanks with the required oxygen content. The inert gas system is not able to produce inert gas and has an inoperable gas temperature sensor. 74 SOLAS 20 II-2/4.5.5.3.1

Ships shall be designed, constructed and maintained with mechanical requirements of a classification society which is recognized by the administration. In the cargo pump room, the stripping pup is leaking cargo from a seal around the drive shaft and has acc accumulated cargo on the deck below the stripping pump. 74 SOLAS 04 II-1/3-1

The IG System shall be capable of delivering inert gas with an oxygen content of not more than 5% by volume to the cargo tanks. Vessel IG System not functioning properly. Unable to prove performance test in presence of [name withheld] restricted. Submit class report to CG to clear deficiency. SOLAS II-2/4.5.5.3.2 FSS Code 15.2.2.1.2.5

In tankers, electrical wiring, equipment, cables shall not be installed in hazardous locations unless it conforms with standards not inferior to those acceptable to the organization. exlosion proof lighting found broken and exposed in cargo room. SOLAS II-1 / 45.11

A main electrical lighting system shall provide illumination to pars of the ship normally used by crew. Two overhead lights found inoperable in AB seamen staterooms.

Subject to the provisions of paragraph 4 and 5 of regulation 3 of this Annex. Oil tankers of 150 gross tonnage and above shall be equipped with an oil discharge monitoring and control system approved by the administration. PSCO observed "sheen" coming from vessel near ODME discharge port. Crew suspected faulty valve in ODME system piping causing pollution from vessel. Request repairs be made and class verify proper opreation of all manual and automatic valves, check for proper operation and structural integrity of entire ODME system and components. For tankers of 20,000 tonnes deadweight and upwards constructed on or after 1 July 2002 but before 1 January 2016, the protection of the cargo tanks shall be achieved by a fixed inert gas system in accordance with the requirements of the Fire Safety Systems Code. Inert gas system includes inert gas systems using flue gas, inert gas generators, and nitrogen generators and means the inert gas plant and inert gas distribution together with means for preventing backflow of cargo gases to machinery spaces, fixed and portable measuring instruments and control devices. The vessel'sinert gas system was unable to calibrate to the level of oxygen required. 74 SOLAS (20) II-2/4.5.5.1 & FSS Code 15.2.1.2

Main propulsion machinery and all auxilliary machinery essentialto the propulsion and safety of the ship shall, as fitted in the ship, be designed to operate. Main engine cylinder #5 exhaust valve air cylinder actuator non-return valve was inoperable. 74 SOLAS (04) II-1/26.6

BALLAST PUMP TEMPERATURE SENSOR HOUSING DETERIORATED AND CRACKED EXPOSING LIVE WIRES IN VESSELS CARGO PUMP ROOM. 74 SOLAS (04) II-1/45.10.4

0635: Before a ship leaves port and at all times during the voyage all lifesaving appliances shall be working and ready for immediate use. Rescue boat davit not raising as designed. Crew must apply pressure to brake in order to raise. Boat aft limit switch fell off during operations, Electrician replaced, 74 SOLAS (14), III/20.2, 60ac.

1835: Tank vessels 20,000 deadweight tons and above shall have IGS in accordance with FSS Code. System not performing as designed. IGG/IGS shut down during operation; crew cannot deduce why and attempted to troubleshoot for 1 hour with no success. #1 blower spraying water. Electrician tampering with setting no valves, 74 SOLAS (140 II-2/4.5.5.1.1, 60ac.

2050: Machinery, Boilers and other pressure vessels; associated piping systems and fittings shall be of a design and construction adequate for the service for which they are intended. Patches found on S/W cooling line to atmosphere pump, flange leaking by sounding pipe for sludge tank, leakage on harbor cool S/W vacuum pump at flange, 74 SOLAS (14) II-1/26, 17ac.

2510: Vessel must maintain ballast water management plan that has been developed specifically for the vessel and is understood by those who follow it. Not all items have been addressed and crew is not properly filling out Bio-Fouling / Ballast Water forms, 33CFR151.2050(g), 40ac.

2050: Means shall be provided to control leakage of flammable material. All generators have oil leakage that drips to the deck below and platform in front of emergency escape, posing a slipping / safety hazard, 74 SOLAS(14), II-2/4.1.1, 40ac.

2510: Marine sanitation device must be operated and maintained in accordance with the manufacturers instructions, patch found on #3 cleaning line, 33CFR159.121(b), 40ac.

1710: The Oil Record Book part 1 shall be completed on each occasion on a tank to tank basis. Weekly soundings for Dirty Lube Oil Tank were logged with incorrect capacity based off outdated IOPP form B from 2012. Incorrect entry dates back to May 2015, logging the capacity as 5.17 instead of 5.7, MARPOL Annex 1 /17.2, 10c.
