INDEPENDENCE OF THE SEAS

Fire doors in main vertical zone bulkheads, galley boundaries and stairway enclosures, the approximate time of closure for hinged fire doors shall be no more than 40 s and no less than 10 s from the beginning of thier movement. FD TT.7.15 did not close fully after 40 s. Additionally, multiple fire doors at the boundaries between main vertical zones closed less than 10 s. 74 SOLAS (2020) II-2/9.4.1.1.5.2 Code 40ac

The following functional requirements shall be met: escape routes shall be maintained in a safe condition, clear of obstacles. Engress from the suite lounge was blocked by a sofa. 74 SOLAS (2020) II-2/13.1.2 Code 10c

The means of escape, including stairways and exits, shall be marked by lighting or photoluminescent strip indicators placed not more than 300 mm above the deck at all points of the escape route, including angles and intersections. The marking must enable passengers/crew to identify the routes of escape and readily identify the escape exits. Additionally, escape route signs and fire equipment location markings shall be of photoluminescent material or marked by lighting. In MVZ 8 stairway between deck 1 and 0, multiple low location light bulbs were not lit. 74 SOLAS (2020) II-2/13.3.2.5.1 Code 10c

The construction of doors and door frames in "A" class divisions, with means of securing them when closed shall provide resistance to fire as well as to the passage of smoke and flame equivalent to that of the bulkheads in which the doors are situated. Fire door 7.02 between incinerator room and engine space would not maintain the boundary as door remained open due to ventilation. 74 SOLAS 20 II-2/9.4.1.1.2

Fire extinguishers shall be situated ready for use at easily visible places which can be reached quickly and easily at any time in the event of a fire... PSCOs observed locker 3.8.3 in the main galley which was incapable of being opened. This locker contained the portable fire extinguisher for this space. 74 SOLAS 20 II-2/10.3.2.4

Escape door from public spaces that are normally latched shall be fitted with a means of quick release. Such means shall consist of a door latching mechanism incorporating a device that releases the latch upon the application of a force in the direction of flow...3 Not be equipped with any locking device, set screw or other arrangement that prevents the release of the latch when pressure is applied to the releasing device. PSCOs observed two escape doors (4.7.6 & 4.7.3) that had installed locks that could prevent the release of the latch when pressure is applied. 74 SOLAS 20 II-2/13.3.2.6.2

Fire Door 7.02 between incinerator room and engine space would not maintain the boundary as the door remained open. 74SOLAS20 CH. II-2 / REG. 9.4.1.1.2

PSCO's identified low location lighting that was out in front of the Johnny Rockets Galley, Deck 12 forward. Emergency escape signage was absent from the Carpenter workshop, Deck 1 forward. 74SOLAS20, CH. II-2 / REG. 13.3.2.5.1

Every crew member with assigned emergency duties shall be familiar with these duties before the voyage begins. Liferaft deployment team were not familiar with the procedures to safely deploy, inflate, and launch a liferaft. 74SOLAS20 CH. III / REG. 19.2.1

Crew members shall be trained to be familiar with the arrangements of the ship as well as the location and operation of any fire-fighting systems and appliances that they may be called upon to use. One mess cook in the Playmakers Galley was not familiar with the manual release for the HI-FOG system or how to extinguish a fire in the deep fat fryer using a portable extinguisher or where the portable extinguisher is located. 74 SOLAS (20) II-2/15.2.2.1

PSCOs observed low location lighting not functioning properly on deck 8 near cabin 8336 and on deck 10 near cabin 1340. 74 SOLAS (20) II-2/13.3.2.5.1

11129 - Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be be in working order and ready for mmediate use. Life boats #12 and 14 could not swing the rudder to the full range of motion. Life boat #12 was corrected on the spot. #14 could not be fixed and remained slightly reduced maneuverability to stbd. 74 SOLAS 20 III/20.2

11110 - Eery life raft shall be stowed with its painter permanently attached to the ship. Life raft on stbd aft of embarkation deck had the painter line loose and on the deck. Painte must be connected to the weak link of the hydrostatic release. 74 SOLAS 20 III/13.4.1

11129 - Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. Lifeboat #7's canopy light had a broken mount but was still operational. 74 SOLAS 20 III/20.2

11131 - Every crew member with assigned emergency duties shall be familiar with these duties before the voyage begins. PCSO observed multiple stairways guides assigned to crew stairway 5A starboard were not familiar with actions to take in an emergency, nor ship's communication procedures during an emergency. 74 SOLAS 20 III/19.2.1

07101 - Spaces are classified according to their fire risk as shown in catagories (1) to (14). A/C space on deck 2 near stairway 2C port is a catagory 10 space, where storage of combustibles is prohibited. PSCO observed multiple cardboard boxes of A/C filters in the space. 74 SOLAS 09 II-2/9.2.3.3.2.2

Seafarers shall receive safety familiarization and basic training or instruction. Crew members assigned to aft galleys in deck 2, 3, and 5 were unfamiliar with the location and operation of manual call points, fire extinguishers, and ventilation shutdowns. Crew in coffee promenade and Sorrento's in deck 5 were also unfamiliar with the location and operation of the fire fighting equipment previously mentioned. STCW 2017 VI/1.1

07120 - The means of escape, including stairways and exits, shall be marked by lighting. Low location lighting throughout multiple decks on stairwell 5A starboard was blocked by luggage. 74 SOLAS 09 II-2/13.3.2.5.1

07105 - The construction of door frames in A class divisions, with a means for securing them when closed, shall provide resistance to fire as well as to the passage of smoke and flame the equivalent to that of the bulkheads in which the doors are situated. A class fire doors 1.2.03, TT.7.15, and 1.5.16 did not latch properly. 74 SOLAS 09 II-2/9.4.1.1.2

07103- Fire Safety- The minimum fire integrity of all bulkheads and decks shall be as prescribed in tables 9.1 and 9.2. Two holes were observed in the A-0 boundary. SOLAS 09 II-2/9.2.2.3.1

07105 - Fire Safety - Fire doors in main vertical zone bulkheads shall be self-closing and be capable of closing with an angle of inclination of up to 3.5 degrees opposing closure. Fire door 1.1.1 in crew stairway did not latch properly. SOLAS 09 II-2/9.4.1.1.4.1

11- Life Saving Appliances- Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. PSCOs observed broken bilge selector switches on the bilge pumping system on lifeboats 1 and 18. SOLAS 20 III/20.2

13199- Other (machinery): Flexible fuel pipes and end attachements shall be of approved fire-resisting materials of adequate strength and shall be constricted to the satisfaction of the administration. Flexible fuel pipes on the emergency diesel generator found to be deteriorated. Solas 09 II-2/4.2.2.5.1

11134- Operation of Life Saving Appliance: Every Crew member with assigned emergency duties shall be familer with these duties before the voyage begins. Several lifeboat commanders were observed to be unfamiler with lifeboat emergency systems to include emergency steering, fuel shutoff, and bilge de-watering systems. Solas 20 III/19.2.1

9101- Safety In General- Times for submitted NOAs are as follows: if your voyage time is less than 96 hours then you must submit a NOA before departure but at least 24 hours before arriving at the port or place of destination. The P/V INDEPENDENCE OF THE SEAS entered Port of Miami on January 26, 2021 without submitting a notice of arrival in accordance with 33CFR 160.212(a)(4)(ii)

07210- Means of Escape- Doors in escape routes shall, in general open in the direction of escape. Found escape route to suite lounge has a door that does not open in way of escape. -74SOLAS(14 Cons.)II-2/13.3.1.5

07120-Means of Escape- Escape routes shall be maintained in a safe condition, clear of obstacles. Secondary escape route forward from suite lounge is obstructed by chairs and tables. -74SOLAS(2014 Cons.)II-2/13.1.2

Every crew member with assigned emergency duties shall be familiar with these duties before the voyage begins. Several galley workers were unfamiliar with ships installed fire fighting equipment and procedures. SOLAS 2014 III/19.2.1 - 10c

Escape route lighting is not working properly in the following locations-RCTV office deck 4, assembly station light near FD 1.6.15 deck 1, assembly station light in olive twist 14, assembly light deck 13 sports court, and exit light forward lobby deck 11. 74 SOLAS (09 CONS.) CH.II-2/13.3.2.5.1. Action Taken 16ac.

Secondary escape route in the conference room located on deck 10 as indicated on fire control is blocked off by structural fire protection. Means of escape from photo lab is locked and cannot be opened without a key. 74 SOLAS (09 CONS.) CH. II-2/13.1.2. Action Taken 16ac.

Stairways and exits, shall be marked by lighting or photo luminescent strip indicators. Multiple areas throughout the ships low location lighting is not working properly. 74 SOLAS (09 CONS.) CH.II-2/13.2.5.1. Action taken 16ac.

Passenger ships carrying more than 36 passengers shall be equipped with an automatic sprinkler of an approved type complying with the Fire Systems Safety Code. Multiple ampoules for sprinklers in the laundry spaces are empty, rendering the system ineffective.74 SOLAS (09 CONS.) CH.II-2/10.6.1.1. Action taken 40ac.

"A" class divisions are constructed as to be capable of preventing the passage of smoke and flame to the end of the one-hour standard fire test. Multiple cables running through “A” class boundary located between main engine room to diesel generator room 2. 74 SOLAS (09 CONS.) CH.II-2/3.2.4. Action Taken 16ac.

Spaces are classified according to their fire risk as shown in caragories 1 to 14; space shall be treated as a space with the releveant category hacin ghte most stringent requirements. Pool Equipment Room on Deck 10 is categorized as a Category 10 Space, but contains equipment which make it a Category 13 Space Workshop. SOLAS (04) II-2/9.2.2.3.2.2
