The High-level alarm required by paragraph (a) of this section must activate a visible and audible alarm so that it can be seen and heard on the vessel where cargo transfer is controlled. High level alarm fuse failed in cargo control room, alarm was not sounding as a result. Source of short suspected to be water intrusion into system.
Condition: Improper/Lack of Maintenance
Action required: 40 - Rectify deficiencies prior to next US port after sailing foreign
