07126 - Means shall be provided to control leaks of flammable liquids. PSCO observed fuel oil collecting in main engine void space starboard side lower deck, and in metal pans under Number 2 and 3 generator. Oily rags were found in multiple void spaces around main engine. Additionally PSCO observed plastic containers with fuel oil in purifier room.

The machinery, boilers and other pressure vessels, associated piping systems and fittings shall be of a design and construction adequate for the service for which they are intended and shall be so installed and protected as to reduce to a minimum any danger to persons on board. Incinerator was missing bricks, exposing the housing.

0720- FIRE FIGHTING EQUIPMENT - The function of fixed firefighting detection and fire alarm system shall be periodically tested to the satisfaction of the Administration by means of equipment producing hot air at appropriate temperature to which the detector is designed to respond. PSCO observed UV lamp was INOP and crew were testing with controlled burn of paper. Vessels crew couldn't provide evidence that this is an approved means of testing flame detectors.
