The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. PSCO observed steady water leak from the #1 main engine cylinder. This jacket water leak was reported to be caused by a worn o-ring. Observed issue could cause loss of cylinder compression and main propulsion loss, presenting a clear hazard to the ship, port and crew.

These guidelines require incinerator walls to be protected by bricks/refractory to maintain satisfactory temperatures. PSCO observed brick/refractory on incinerator door interior to be heeavily deteriorated and worn through reducing thermal and fire protection.
