The owner, master, or person-in-charge of a tank vessel must ensure that portable and semi-portable extinguishers are inspected and maintained in accordance with NFPA 10 (incorporated by reference, see § 31.01–2) as specified in paragraphs (a)(1) through (4) of this section. The owner or managing operator must provide satisfactory evidence of the required servicing to the marine inspector. If any of the equipment or records has not been properly maintained, a qualified servicing facility must perform the required inspections, maintenance procedures, and hydrostatic pressure tests. A tag issued by a qualified servicing organization, and attached to each extinguisher, may be accepted as evidence that the necessary maintenance procedures have been conducted. All portable extinguisher on deck are not charged and the material condition of the bottles have excessive corrosion. Service extinguishers and provide technician report to the satisfaction of the attending marine inspector.

Each cargo tank of a tank barge must have one of the following liquid overfill protection arrangements: An intrinsically safe overfill control system that activates an alarm and automatic shutdown system at the facility overfill control panel 60 seconds before the tank is 100 percent liquid-full during a facility-to-vessel cargo transfer. Vessel unable to demonstrate proper operation of high level/overfill alarm system.

The material, design, construction, workmanship, and arrangement of main propulsion machinery and of each auxiliary, directly connected to the engine and supplied as such, must be at least equivalent to the standards established by the ABS Steel Vessel Rules (incorporated by reference, see 46 CFR 58.03–1), except as otherwise provided by this subchapter. The number 1 had fuel leaks on the fuel pump and fuel lines to the engine. The # 2 generator had multiple fuel line leaks at the connections to the engine. Inspect all fuel lines and identify all that are leaking and make appropriate repairs to the satisfaction of the attending marine inspector.

The vessel's bow port and starboard running light screens were wasted with rust and not painted flat black as required by the 72 COLREGS Annex I positioning and technical detail of lights and shapes. Must be painted flat black prior to night-time navigation.

The last 1.0 meter (3.3 feet) of vapor piping upstream of the vessel vapor connection and each end of a vapor hose must be Labeled with the word “VAPOR” painted in black letters at least 50.8 millimeters (2 inches) high.

Each vessel must carry lifebuoys approved under approval series 160.150 as follows: Lifebuoys must be fitted with a buoyant lifeline that is of a breaking strength which is not less than 5 kiloNewtons (1,124 pounds-force). Lifebuoys stowed on the port side aft storage space had wasted lifelines that require replacement.

Ventilator are protected by the trunk by watertight covers, found section of gaskets missing for the following intake and exhaust trunk covers, preventing spaces form being sealed in the event of a fire: a. Port and Starboard intake air plenum diesel generators b. Port and starboard generator exhaust trunk c. Port and starboard cargo pump room intake and exhaust Cargo restricted to Grade D and lower.

All safety and relief valves for use on pressure vessel or piping systems shall be designed to meet the protection and service requirements for which they are intended and shall be set to relieve at the pressure which does not exceed the maximum allowable working pressure. Found the #1 start air compressor relief valve was inoperable in that it would not operate when hand relieved.

Battery operated lanterns must be capable of providing light for at least 3 hours and must be relay-controlled so that the loss of normal power causes the lanterns to light. Found five (05) battery operated emergency lights were inoperable.

The requirements of 58.10-5(a) , (c) and (d) shall apply to diesel engine installations. Found that the starboard cargo pump diesel engine muffler had an excessive exhaust leak. Engine to be tagged out of service until muffler replaced.

When inspecting oil-burning vessels, either internal-combustion type or steam-driven type, the marine inspector shall examine the tank tops and bilges in the fireroom and engineroom to see that there is no accumulation of oil which might create a fire hazard.

Each lifebuoy must be marked in block capital letters with the name of the vessel and the name of the port required to be marked on the stern of the vessel under § 67.123 of part 67 of this chapter

The equipment of all tank vessels, such as fire hose, fire axes, lifeboats, life rafts, life preservers, and lifeboats, shall be painted or branded with the name of the vessel upon which they are used. Found that the lifejackets were not marked with the with the vessel name and home port.

The vessel stern port and starboard running light screen were not painted flat black as required by the 72 COLREGS Annex I positioning and technical detail of lights and shapes.

Electric installations on vessels must ensure protection of passengers, crew, other persons, and the vessel from electrical hazards. When using a outlet tester found several electrical duplex 120 AC outlets were incorrectly wired

On a tank barge, each cargo tank must be equipped with an overfill device that meets the requirements of 46 CFR 39.2007(b)(2) through (b)(4), (d)(1) through (d)(4), and 46 CFR 39.2009(a)(1) . Found the overflow audible alarm was not operable.

Electric installations on vessels must ensure:Maintenance of system integrity through compliance with the applicable system requirements (IEEE, NEC, IEC, etc.) to which plan review has been approved, and the termination of each cable must be sealed to prevent the entrance of electrolyte by spray or creepage. Found both battery terminal connectors for the #1 Generator where of a wing nut design (non-solder type) and showed signs of signs of corrosion from electrolyte. 46 CFR 111.01(a)(3) and 46 CFR 111.15-20

Each vessel must have posters or signs displayed in the vicinity of each survival c that - (a) Illustrate the purpose of controls; (b) Illustrate the procedures for operating the launching device; (c) Give relevant instructions or warnings; (d) Can be easily seen under emergency lighting conditions; and (e) Display symbols in accordance with IMO Resolution A.760(18). The placards for the liferaft launching procedures were deteriorated due to uv damage.

The ring, lines and letters shall be painted in white or yellow on a dark ground or in black on a light ground. They shall also be permanently marked on the sides of the vessels to the satisfaction of the assigning authority. The marks shall be plainly visible and, if necessary, special arrangements shall be made for this purpose. The port load line marking was not visible due to deterioration of the paint.

Draft marks must be painted in contrasting color to the hull. Found the paint for the Port and Starboard draft marks on the stern of the barge deteriorated.

Each outlet or junction box must be securely attached to its mounting and be affixed so as to maintain its designated degree of protection. Found in the aft store room that a marine grade duplex outlet box was hanging by the electrical cables, Located on starboard side of the door to the weather deck.

Certificates of Inspection will be issued for a period of 5 years; COI expired 05MAY2020 Complete COI Inspection prior to conducting cargo operations.

USCG approval letter for SOPEP was expired. Acquire new, valid USCG approval letter.

Liferaft hydrostatic release units were not properly rigged.

Vessel did not have a copy of Benzene Monitoring Program on board.

Any oil tanker involved in STS operations shall carry on board a plan prescribging how to conduct STS operations (STS operations Plan) not later than the date of the first annual, intermediate, or renewal survey of the ship to be carried out on or after 1 January 2011. Vessel did not have a STS operations Plan on board in accordance with its IOPP Form B. Acquire STS operations plan.

Deck line is not present.

Non approved light installed within hazardous zone surrounding pressure vacuum valve.

Vessel did not have EIAPP certificates on board for any engines. 40 CFR 1043.30(c)

Make permanent repairs to the damaged areas in the forward rake to the satisfaction of Class and the attending USCG MI.

The attending Marine inspector was unable to conduct overspeed and low lube oil shutdowns on the generator. Make generators available for testing in the presence of a Marine Inspector.

Replace/Provide light bulbs for ground detection on the L100 panel in the generator space, and the P100 panel in the office. If ground is present, rectify the ground.

Aft generator failed to conduct overspeed shutdown. Display proper operation of generator and have marine inspector witness proper testing of overspeed shutdown.

Prior to the carriage of cargo, make proper hull repairs to the hull inset approx.5'x5' located at frames 37 & 38 IWO 4S wing void.Repairs are to the satisfaction of the attending CG MI and ABS. Monitor the NO.4S wing void during watchstander rounds/every hour. ANy changes to the void condition contact the local OCMI Zone.

Bottles past due for servicing.

Original Certificate of Documentation must be on board.

Provide Certificate of DAPI Enrollment for 2017.

Provide NOx technical files and EIAPP Certificate on board.

Two vapor pressure gauges installed on the port manifold were broken/out of calibration. Replace or service prior to using the affected vapor connection. 46 CFR 39.2015

The signs conspicuously marking the emergency shutdown locations were faded and unreadable. Replace with new signs or properly stencil the signs.

The forward masthead light was extinguished and did not illuminate, despite replacing the bulb. Service light and/or wiring.

A ground was present on the 110V distribution panel. Isolate ground, tag-out affected equipment, and perform necessary repairs.

Replace aft ring buoy. Ring buoy had degraded and is unserviceable

Re-stencil in block letters all ring buoys. Marking is illegible.

Repaint sidelight screens for the port and starboard running lights in matte black. The screen's paint is wasted and the primer is showing.

Prove proper operation of safety rlf vlv on 200 gal air receiver tank, tank not on COI. Safety rlf vlv on the tank, set at 200 psi, could not be tested in place during inspection because the system was protected by a safety relief valve set at 175 psi after each air compressor - for a total of 2 relief valves. Additionally, data plate on vlv was worn.

Clean knife edge and gasket of forward watertight door leading to fwd rake compartment. Edge and gasket were painted, gasket aslo had tape on it, preventing proper seal. Chalk test gasket after cleanning.

Provide records of all crew members enrolled in the American Maritime Safety random drug testing program. List was not on board at time of inspection.

Provide copy of management information system report. Report not on board.

Provide calibration certirficate on both cargo pump discharge gauges.

Life jackets does not have the required lights. Prior to sailing coastwise.

Vessel does not have an approved VOC management plan onboard.
