Fire detection systems shall be in good order so as to ensure their required performance if a fire occurs. PSCO observed heat detector 603095 on deck 4 in the crew galley permanently activated when the galley staff is using cooking appliances. This practice bypasses the use of fire detection within that zone.

PSCO observed A-class fire doors (double leaf) 4523, 4307, 4303, 4417, 4507 on deck 4 did not close correctly (sequence/latch) when tested.
