Periodic Safety tests must demonstrate the proper operation of the primary and alternate controls, alarms, power sources, transfer override arrangements, interlocks, and safety controls. MI found both MDE's low L/O psi shutdown's outside of parameter IAW approved PSTPs.

Complete but simple instructions for the operation of the systems must be located in a conspicuous place at or near all pull boxes, stop valve controls and in the CO2 cylinder storage room. On systems in which the CO2 cylinders are not within the protected space, these instructions must also include a schematic diagram of the system and instructions detailing alternate methods of discharging the system should the manual release or stop valve controls fail to operate. Each control valve to branch lines must be marked to indicate the related space served. MI thoroughly read posted instructions and determined that the listed local operating instructions may inadvertently bypass all installed safety and shut down systems. Vessel operator is required to have the required instructions validated by the servicing company and corrected as necessary. The issuing office understands that the servicing company is scheduled to attend vessel on 07FEB23. 46CFR76.15-10 16c.

On vessels required by 112.05-5(a) to have a temporary emergency power source, the following emergency lighting and power loads must be arranged so that they can be energized from the temporary emergency power source: Lighting, including… muster stations. There was no emergency lights installed on the aft deck in the vicinity of the muster station for the IBAs. 46 CFR 112.15-1(b)

General emergency alarm signal must: Be located in passenger and crew quarter areas where they can alert persons in spaces. The main deck port and starboard general alarms were not operating at the time of inspection. 46 CFR 113.25-9(a)

Internal combustion engine exhausts, boiler and galley uptakes and similar source of ignition shall be kept clear of and suitably insulated from any woodwork or other combustible matter. The starboard main engine exhaust has missing lagging, exposing hot piping. 46 CFR 72.03-10

If your vessel’s Certificate of Inspection is renewed annually, the marine inspector must check the stings of the safety or relief valves on all pressure vessels, except cargo tanks, at each inspection for certification. The vessel has three air receivers on board with three separate relief valves. There is no record of the relief valves having been tested to their set pressure. Additionally one of the relief valves in the engine room was painted over. Provide reports attesting to the accuracy of the relief valve’s set pressures. 46 CFR 61.10-5(i)

Each machinery space must be ventilated to ensure that, when machinery or boilers are operating at full power in all weather including heavy weather, an adequate supply of air is maintained for the operation of the machinery and for the safety, efficiency, and comfort of the crew. The port engine room supply fan is currently not operational. The crew has been opening an access window on the port side of the superstructure to allow air supply to the engine room. 46 CFR 58.01-45

Drills must, as far as practicable, be conducted as if there were and actual emergency. During the man overboard drill, the crew did not follow the vessel’s station bill located in the Approved Shipboard Safety Management and Contingency Plan. Specifically, the crew did not keep communications with the master, nor did they designate a lookout to monitor the location for the man overboard as stated in the plan. The crew, along with attending marine inspectors, reviewed the station bill and requirements for each crew member during the drill. Corrected on the spot. 46 CFR 199.180(c)

Each passenger vessel must have a public address system capable of broadcasting separately or collectively to the following stations:(g) Accommodation spaces and service spaces. Main deck accommodation space does not have a P/A system speaker and cannot be heard when general alarm is activated or announcements are made. Vessel's company shall rectify in a way as the general alarm and P/A system can be heard by passengers in accommodation space when activated.

"All lifesaving appliances and arrangements on passenger vessels must be IAW 46 CFR Subchapter W (Lifesaving Appliances & Arrangements). Weekly inspections and tests: Each survival craft, rescue boat, and launching appliance must be visually inspected to ensure its readiness for use; Each lifeboat engine and rescue boat engine must be run ahead and astern for 3 minutes; The general alarm system must be tested." Upon talking with the master, crew, and company security officer, weekly visual checks of the general alarm, rescue boat and launching appliance, and the 4 on board inflatable liferafts are being done at variable intervals, but not being done at these required weekly intervals. Only logbook entries are seen are at monthly intervals, most two recent entries were 4MAR2020 and 9FEB2020. Conduct weekly visual inspections of all stated lifesaving items & weekly rescue boat engine test and appropriately log, as weekly checks would have identified the 4 liferaft hydrostatic release units were 1 month expired (Feb 2020). Deficiency will stay open for 3 months to be verified at the next quarterly reinspection. Ensure all Masters are aware of this. 46 CFR 70.28-1 and 46 CFR 199.190(d) and (e)

"Each lifesaving appliance, including lifeboat equipment, must be inspected monthly using the checklists required under paragraph (b)(1)(i) of this section to make sure the appliance and the equipment are complete and in good working order. A report of the inspection, including a statement as to the condition of the equipment, must be recorded in the vessel's official logbook." Upon talking with the master, crew, and company security officer, they stated there are no monthly lifesaving appliance inspection checklists for the rescue boat, rescue boat launching appliance (davit), and 4 inflatable liferafts. Create three separate & detailed checklists to be used for the inspection of these different lifesaving equipment items. Furthermore, the 4MAR2020 and 9FEB2020 logbook entries are related to lifesaving drills, not the actual equipment/inventory checks, ensure this monthly check is conducted and properly logged. Create these checklists and provide them for our review no later than 12MAR2020. 46 CFR 70.28-1 and 46 CFR 199.190(d) and (e)

Prior to getting this vessel underway all missing void hatch gaskets must be replaced, gaskets that are not serviceable must be replaced as well. Excessive waster is to be removed for the void spaces where a lack of a gasket caused water to enter the space

Prior to getting the vessel underway, all sounding tube caps on the main deck must be made usable, currently many cannot be opened and have not been opened for quite some time. Also on the main deck the caps for the bilge and ballast suction valves must be made usable.

Each generator must have an overspeed device that is independent of the normal operating governor and adjusted so that the speed cannot not exceed the maximum speed by more than 15 percent. Port generator overspeed device failed to shutdown engine completely. Generator ran at 600 RPM's (without load) while air shutdown was engaged.

Each equipment-grounding conductor must be sized in accordance with Section 250.122 of NFPA NEC 2002. STBD generator is missing grounding strap. Install appropriate grounding strap.

Crew could not follow the posted instructions. Need to either update the instructions to make it easier to follow or train the crew on the differences on the two different sets of instructions.

Remove fluids in e/r batt install. Repair holed batt box aft of bridge.

Replace pressure gauge would not display the MAWP of the pv or the relief valve settings.

Stbd SS Generator reported OOC. Prior to placing back in service, MI must attend for partial survey.

Shutoff valve installed between electric air compressor and the air reciever, thereby leaving a section of the supply piping unprotected (relief valve on compressor was set higher than the air reciever).

Missing 1 required adult lifejacket from Stbd side forward stowage box - only 49 in box marked for 50. Replace the missing life jacket.

Machinery space GA light not working
