Fire doors in main vertical zone bulkheads, galley boundaries and stairway enclosures other than power-operated watertight doors and those which are normally locked shall satisfy the following requirements:the doors shall be self-closing and be capable of closing with an angle of inclination of up to 3.5 degrees opposing closure. PSCO identified more than 8 self-closing doors that did not meet the self-closing requirement including doors in the engine room, the laundry room, and the crew fire locker on the main deck. 74 SOLAS (04) II-2 /9.4.1.1.4 17ac

Electrical installations shall be such that the safety of passengers , crew and ship from electrical hazards will be ensured. PSCO observed exposed wiring going to the AC unit for the wheelhouse. 74 SOLAS (04) II-1/40 17ac

The machinery, boilers, and other pressure pressure vessels, associated piping systems and fittings shall be of a design and construction adequate for the service for which they are intended and shall be installed and protected as to reduce to a minimum any danger to persons on board. PSCO identified a leak in the steam system for jacketwater heating for the main engine. 74 SOLAS (04) II-1 /26.1 40ac

The first non-return device shall be a deck seal of the wet, semi-wet, or dry type or a double block and bleed arrangement. Two shut off valves in series with a venting valve in between, may be accepted provided: the operation of the valve is automatically executed. Signals for opening/closing are to be taken from the process directly and alarm for the faulty operation of the valves is provided. PSCO identified the actuator valves for the nitrogen generator double block and bleed to be severely corroded and operational only manually. Cylinders must always be secured and, when not in use, they must be stowed in a rack in an upright position, with the valve protection cap in place. PSCO observed full oxygen and acetylene cylinders in store rooms and medical oxygen in the hospital space improperly stored on directly on the deck and fastened with rope. 46 CFR 147.60(b)(1) 40ac

Following installation, the master, owner, operator, agent, or person in charge of the vessel must properly maintain the BWMS in accordance with all manufacturer specifications. Vessel's ballast water management system is inoperable. Vessel shall not discharge ballast water into U.S. waters. 33 CFR 151.2025(a)1 40ac

In addition to the requirements contained elsewhere in the present regulations, ships shall be designed, constructed and maintained in compliance with the structural, mechanical and electrical requirements of a classification society which is recognized by the Administration in accordance with the provisions of regulation XI/1, or with applicable national standards of the Administration which provide an equivalent level of safety. PSCO observed extensive corrosion on exterior decks, bulkheads, ladder ways, guardrails, closures, and piping. PSCO identified temporary patches and wastage on the cargo tank steam lines, tank cleaning lines, and the eyewash station water supply lines. 74 SOLAS (04) II-1/3-1 40ac

After any survey of the ship under regulations 7,8,9, or 10 has been completed, no change shall be made in the structural arrangements, machinery, equipment and other items covered by the survey, without the sanction of the Administration. PSCO observed designated lifebuoy bracket without lifebuoy. Vessel replaced the lifebouy. Deficiency rectified. 74 SOLAS (20) I/11(b) 40ac

A' class doors shall be constructed as to be capable of preventing the passage of smoke and flame. Stairwell self-closing door was found to be non-self-closing.

Each vapor collection flange must have a stud projecting outward located at the top of the flange face. Vapor lines on port/starboard manifolds lack required stud.

The last meter of vapor piping must be painted in the sequence of red/yellow/red. Vapor lines on port/starboard manifolds lack required markings.

Means shall be provided to control leaks of flammable liquids. PSCO observed accumulation of oil around #1 piston head on the #1 generator. Vessel is aware of inoperable gasket, requisition order submitted. 74SOLAS(09)II-2/4.1.1

All steering gear components and the rudder stock shall be of sound and reliable construction to the satisfaction of the adiministration. PSCO observed hydraulic oil leaks from both pumps of steering gear system, with unknown source. 74SOLAS(04)II-1/29.2.1

The fire integrity of the divisions shall be maintained at openings and penetrations. PSCO observed 02 self closing doors that were not capable of closing on their own. 74SOLAS(09)II-2/9.1.3
