Fire doors in main vertical zone bulkheads, galley boundaries and stairway enclosures other than power-operated watertight doors and those which are normally locked shall satisfy the following requirements:the doors shall be self-closing and be capable of closing with an angle of inclination of up to 3.5 degrees opposing closure.
PSCO identified more than 8 self-closing doors that did not meet the self-closing requirement including doors in the engine room, the laundry room, and the crew fire locker on the main deck.

The first non-return device shall be a deck seal of the wet, semi-wet, or dry type or a double block and bleed arrangement. Two shut off valves in series with a venting valve in between, may be accepted provided: the operation of the valve is automatically executed. Signals for opening/closing are to be taken from the process directly and alarm for the faulty operation of the valves is provided.
PSCO identified the actuator valves for the nitrogen generator double block and bleed to be severely corroded and operational only manually. The valves could not be automatically executed.

PSCO observed extensive corrosion on exterior decks, bulkheads, ladder ways, guardrails, closures, and piping. PSCO identified temporary patches and wastage on the cargo tank steam lines, tank cleaning lines, and the eyewash station water supply lines.
