Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons onboard. Marine inspector discovered center shaft through hull penetration tube with unauthorized metal repair using soft patch, sealant & hose clamp.

Resolution: Inspected shaft through hull, about a 1.5ft section of pipe was replaced and welded into place. New strut bracing was welded in.

Marine inspector discovered excessively worn pivot points where the hydraulic steering rams connect to lever arms, and associated worn pivot points that drive rudder movements.
