Fire doors in main vertical zone bulkheads, galley boundaries, and stairway enclosures shall be self-closing and be capable of closing with an angle of inclination of up to 3.5 degrees opposing closure. PSCOs observed two fire doors, both leading to crew stairwells, that were unable to latch closed after activation. The doors are FD08705 on deck 8 and FD09604 on deck 9.

Except for hatches between cargo, special category, store, and baggage spaces, and between such spaces and the weather decks, openings shall be provided with permanently attached means of closing whch shall be at least as effective for resisting fires as the divisions in which they are fitted. PSCOs observed one B-15 self-closing door which was capable of closing and latching, however the door's edge is damaged in such a way that it is no longer as effective for resisting fires as the division in which it is fitted. The door is located on deck 4, main vertical zone 6 in front of the ice cream preparation.

Lifebuoys shall be so stowed as to be capable of being rapidly cast loose, and not permanently secured in any way. PSCOs observed the life buoy's lifeline tied to itself in such a way that PSCO was unable to get it unraveled. The lifebuoy is located in front of lifeboat #12.
