The discharges of substances assigned to category X, Y, or Z, shall be prohibited unless such discharges are made in full compliance with applicable operational requirements of this Annex. Paraxylene was discharged out of the Annex II overboard valve from an unknown source while loading. Provide a causal analysis or class report on the repairs or actions taken to ensure Annex II discharges are controlled in the future.
Action required: 50 - Rectify deficiencies w/in 30 days

Resolution: Vessel master presented sufficient evidence of RO oversight of the rectification of the observation. All sat.

Electrical installation shall be such that all electrical auxiliary services necessary for maintaining the ship in normal operational and habitable conditions will be ensured without recourse to he emergency source of electrical power. The S phase on the 220V feeder AC panel is indicating a major fualt in the circuit.
Condition: Improper/Lack of Maintenance
Action required: 17 - Rectify deficiencies prior to departure
