FIRE PROTECTION SYSTEMS AND FIRE-FIGHTING SYSTEMS AND APPLIANCES SHALL BE MAINTAINED READY FOR USE. PSCO OBSERVED A DELAY IN THE IMMEDIATE ACTIVIATION OF THE EMERGENCY FIRE PUMP BY THE CREW., WHICH WAS ATTRIBUTED TO A MALFUNCTION IN THE FUSE BOX. 74SOLAS(2020 CONS.) The machinery, boilers, and other pressure vessels, associated piping systems and fittings shall be of a design and construction adequate for the service which they are intended and shall be so installed and protected as to reduct to a minimum any danger to persons on board, due regard being paid to moving parts, hot surfaces and other hazards. PSCO conducted damage survey following propulsion irregularity and issue of COTP 339-20 dated 05AUG20. C/E stated prior to departing St. Rose, LA, exhaust temp sensor on #1 ME cylinder was replaced, but found to be faulty and caused reduction in RPMs. #1 ME cylinder exhaust temp sensor has been replaced. 74 SOLAS (2014) II-I/26.1, 10c

74 SOLAS(14) II-2/4.5.5.3.2: THE INERT GAS SYSTEM REFERRED TO IN PARAGRAPH 5.5.3.1 SHALL BE DESIGNED, CONSTRUCTED AND TESTED IN ACCORDANCE WITH THE FIRE SAFETY SYSTEMS CODE. FSS 15.2.3.1.6.1: SPECIAL CONSIDERATION SHALL BE GIVEN TO THE DESIGN AND LOCATION OF SCRUBBER AND BLOWERS WITH RELEVANT PIPING AND FITTING IN ORDER TO PREVENT FLUE GAS LEAKAGES INTO ENCLOSED SPACES. PSCO OBSERVED AN ATMOSPHERE IN THE IGG BELOW ACCEPTABLE LIMITS FOR OXYGEN.

Manual bypass valve on Inert Gas System was fractured when crew member attempted to operate. The casing surrounding the valve stem broke off. The condition of the ship and its equipment shall be maintained to ensure that the ship in all respects will remain fit to proceed to sea without danger to persons onboard. The inert gas plant has a leak in the IG room, creating an unsafe environment for personnel. Carbon monoxide reading from outside the space was measured at 50 ppm. Source of the leak is unknown. 74 SOLAS (14) I/11(a) (60a/c)

The inert gas system shall be capable of inerting, purging, and gas-freeing empty tanks and maintaining the atmosphere in cargo tanks with the required oxygen content. The IGG is not operational with re-occurring flame failures. 74 SOLAS (14) II-2/4.5.5.3 (60a/c)

0936 - Steering gear - The condition of the ship and its equipment shall be maintained ito confrom with the provisions of the present regulations. The rudder angle indicator in the steering gear room was showing a difference of 5 to 20 degrees compared to the fixed. 74 SOLAS (14) I/11 50a

1840- Instrumentation- Instrumentation shall be fitted for continuously indicating the oxygen content of the inert gas in the inert gas supply mains on the d/c side of the gas blowers. Oxygen analyzing unit would not stabilize on proper reading. 74 SOLAS(09 CONS) II-2/ 11.6.1

1835- Cargo Vent System- Venting arrangements shall be so designed and operated as to ensure that neither pressure nor vacuum in cargo tanks shall exceed design parameters. Flame screens on the vacuum side of PV's were saturated with cargo. 74 SOLAS(09 CONS) II-2/11.6.1

1430 - Auxiliary engines- The condition of the ship and it's equipment shall be maintained. PSCO observed that during testing of the Emergency Diesel Generator that a common ground fault is alarming to the local bus panel. SOLAS 74AM I/11

Inert gas system was reported by the ship’s crew to not be operating fully and properly during cargo operations.

High O2 content meter not sending visual or audible alarm for O2 levels exceeding 5% to cargo control room, engine control room, or bridge. Vessel is to provide means for a visual and audible alarm for the oxygen content meter of the inert gas system or provide an alternate safe discharging plan in writing to Coast Guard prior to discharging cargo.

The main engine cylinder no. 5 lube oil line was air bound causing the vessel to auto slow down in the Lower Mississippi River.

During inspection of cargo tanks (5P, 3P, and 2P), Oxygen levels were above 8%. Using ship's Inert Gas Generator, sweep cargo tanks to bring oxygen levels below 8% prior to conducting cargo operations.

Shipboard logkeeping was inconsistent with what was being sent to Owner in regards to Cargo Tank Inert Gas Report. The report being sent to owner differed from logs being kept by [name withheld] Have attending surveyor from Recognized Safety Management Organization ensure ship is carrying out proper Cargo IG procedures as per Safety Management Plan (5.03).
