All access openings in bulkheads at ends of enclosed superstructures shall be fitted with doors of steel or other equivalent material, permanently and strongly attached to the bulkhead, and framed, stiffened and fitted so that the whole structure is of equivalent strength to the unpierced bulkhead and weathertight when closed. Identified a hole (from corrosion) in the outboard side of door “810 Upper Deck”. Requested crew to investigate condition of all doors on this deck and noted similar condition on the door directly outboard/port side of this door. ILLC (1966) Reg 12(1)

The training manual, which may comprise several volumes, shall contain instructions and information, in easily understood terms illustrated wherever possible, on the life-saving appliances provided in the ship and on the best methods of survival…The following shall be explained in detail: donning of lifejackets, immersion suits and anti-exposure suits, as appropriate. 3 different types of lifejackets were found on board but only two types were captured in the SOLAS training manual. All electrical apparatus shall be so constructed and so installed as not to cause injury when handled or touched in the normal manner. Identified the following electrical items: -In the Bos’n Locker, multiple power strips were daisy chained together supplying power to multiple items, including recharging stations. Corrected. -A broken light cover in the Fire Locker. Corrected. -Missing a gland seal in the light above the port side Pilot Station. Corrected. -Identified a wasted junction box in upper bow thruster space (next to bow thruster fan vent limited switch). SOLAS (2004) II-1/Reg 45.1.3

The company should establish procedures, plans and instructions, including checklists as appropriate, for key shipboard operations concerning the safety of the personnel, ship and protection of the environment. A “Danger Do Not Operate” lockout tag for the fuel changeover valve was posted on but not filled out. SOLAS (2020) IX/Reg 3, ISM Code Part A Ch.7

Every steam boiler…shall be provided with not less than two safety valves of adequate capacity. The forward boiler safety relief valve did not reseat after pulling the easing gear. Also, the aft boiler safety relief valve was missing the insulation around the valve. SOLAS (2004) II-1/Reg 32.1

On cargo ships, each lifejacket shall be fitted with a lifejacket light complying with the requirements of paragraph 2.2.3 of the Code. Throughout both the aft and forward PFD storage bins two lifejackets had missing lights and three lifejackets had lights that were inoperable.

Not less than one half of the total number of lifebuoys shall be provided with lifebuoy self-igniting lights complying with the requirements of paragraph 2.1.2 of the Code. The starboard side lifebuoy light, first one aft of the gangway, was found inoperable

Fire protection systems shall be kept in good order so as to ensure their required performance if fire occurs: means of escape systems and appliances. • The self-closing fire door from the accommodations stairway to the engine room did not close. • The self-closing fire door for the forward engine room escape did not close/latch.

Fire protection systems shall be kept in good order so as to ensure their required performance if fire occurs: structural fire protection, including fire-resisting divisions, and protection of openings and penetrations in these divisions. The outboard EDG fire damper did not secure when remote closuring device was activated.

The condition of the ship and its equipment shall be maintained to conform with the provision of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board.In the EDG space, the SCBA air compressor was not properly secured in its bracket.

Vessel reports water ingress into bilge holding tank at rate of 0.25 cubic meters/hour from unknown source. Suspected hull breach.

A safety system shall be provided to ensure that serious malfunction in machinery or boiler operations, which presents an immediate danger, shall initiate the automatic shutdown of that part of the plant and that an alarm shall be given. Auxiliary boiler fuel oil regulating valve and fuel controller coupling is faulty. This causes certain boiler automation safeties to not operate properly in normal mode (automatic). Crew is running auxiliary boiler in manual mode only. Crew must complete all lighting off and shutdown procedures as posted at boiler. This coupling has consistently failed and been problematic for the last 5-6 years. Company and ABS were notified on previous occasions. Previous repairs became faulty again in a short period of time. Make permanent repairs to boiler with any recommendations for manufacturer to ensure reliability of the automatic operation of the boiler. Engine room shall be fully manned at any time the boiler is running in manual mode. C/E shall provide standing orders for engineering crew on immediate actions to take in the event of any boiler alarm.

Oil fuel pipes, which, if damaged, would allow oil to escape from a storage, settling or daily service tank having a capacity of 500l and above situated above the double bottom, shall be fitted with a cock or valve directly on the tank capable of being closed from a safe position outside the space concerned in the event of a fire occurring in the space in which such tanks are situated. Quick closing fuel valves did not operate due to low pressure in dedicated air receiver. C/E identified solenoid valve at controls with fittings causing leak by. C/E replaced solenoid, but leak continued. Properly repair system and test all quick closing valves.

An efficient bilge pumping system shall be provided, capable of pumping from and draining any watertight compartment other than a space permanently appropriated for the carriage of fresh water, water ballast, oil fuel or liquid cargo and for which other efficient means of pumping are provided, under all practical conditions. Installed bilge pump and secondary bilge pump were unable to pump bilge pockets. Make repairs and test bilge pumping arrangements.

The purpose of this regulation is to limit the fire growth potential in every space of the ship. The main inlets and outlets of all ventilation systems shall be capable of being closed from outside the spaces being ventilated. The means of closing shall be easily accessible as well as prominently and permanently marked and shall indicate whether the shut-off is open or closed. Galley exhaust ducting is coated in grease/dust. 1 of 2 fire dampers in ducting were found inoperable. Clean all internal ducting and prove proper operation of fire dampers.

External high-pressure double jacketed fuel delivery piping systems shall include a means for collection of leakages and arrangements shall be provided with an alarm in case of a fuel line failure. Main diesel engine fuel oil high pressure piping leak alarm found inoperable. Float system was cleaned.

Electrical installations shall be such that: the safety of passengers, crew and ship from electrical hazards will be ensured. The following electrical items were identified during inspection and require proper approved installation: 1- New additional LED lighting permanently installed throughout engine room and on deck. Some found with unapproved splices, plugged into extension cords or mounted power strips. Some on deck found with broken covers. Permanently installed fan in E/R workshop found plugged into extension cords and plugged into power strip. 2- Numerous unplugged cable fittings found on control cabinets and motor control boxes. 3- Lighting cable at front of MDE on “E” deck exterior port side improperly supported. 4 - No. 1 DG jacket water heater controller and sump tank low level alarm improperly mounted/installed. 5 - Port E/R bilge alarm wiring exposed at junction box. 6 - Dead end wires found at Port E/R bilge pocket and at GMDSS battery locker. 7 - No. 1 L.O. Prime Pump Starter motor controller cabinet does not close properly. 8- 02 emergency lights inoperable outside EDG Room.

All distribution boxes and manually operated valves in connection with the bilge pumping arrangements shall be in positions which are accessible under ordinary circumstances. Reach rods to additional suction valves to engine room that can be used for emergency dewatering were not connected due to new valve installation. New reach rods must be fabricated/installed. Main emergency bilge suction reach rod was found intact.

The number of openings in watertight subdivisions is to be kept to a minimum compatible with the design and proper working of the ship. 03 deck plate openings for access to tank top found with broken hinges. Multiple deck plates found unsecured from yard period. Repair hinges and secure deck plates.

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board.Multiple analog gauges on diesel generators are damaged or inoperable. Replace gauges with approved type or remove if not required by ABS Class Rules.

It is the intent of this subpart to provide such markings as are necessary for the guidance of the person on board in case of an emergency. Mark unmarked alarms sirens/lights with appropriate signage in accordance with all regulations under 46CFR 97.37.

A plethora of loose equipment, spare piping and valves, sheets of steel and other items were found improperly stowed for sea due to shipyard work. Stow all loose equipment and spares for sea.

The maintenance program of [46 CFR] 62.50-20(h) must include a checkoff list to make sure that routine daily maintenance has been performed, fire and flooding hazards have been minimized, and plant status is suitable for unattended operation. Completion of this checkoff list must be logged before leaving the plant unattended. o There was no maintenance program check off list utilized by the crew for unattended plant operation. o No procedure or log entries were notated for completion of the unattended operations checklist in the Engineer’s logbook. o No watch schedule was posted for the engineer’s watches. Measures shall be taken to the satisfaction of the Administration to ensure that the equipment is functioning in a reliable manner and that satisfactory arrangements are made for regular inspections and routine tests to ensure continuous reliable operation. The #3 steering gear pump phase fail alarm did not alarm during test of PTSP. SOLAS (2004) II-1 / Reg 46.2

Cables and wiring shall be installed and supported in such a manner as to avoid chafing or other damage. o Air compressor for SCBA bottles located in EDG room was run with extension cords, a hole drilled through side of emergency switchboard, and extension cord wire was utilized to feed circuit. o In the fire locker, a power strip was modified to support for an electrical cable used to recharge the certified safe torches. SOLAS (2004) II-1 / Reg 45.5.5

An electrical cable splice identified in an emergency-light on forward superstructure port side (below fire locker) and on starboard side main deck lighting near the gangway.

All ships of 500 gross tonnage and upwards shall have...rudder operational mode indicators to determine and display rudder angle. The rudder angle indicator on the starboard bridge wing was weathered and not adequately visible from the control station. SOLAS (2004) V / Reg 2.5.4

Fire protection systems and fire-fighting systems and appliances shall be maintained ready for use. The CO2 valve box on the cargo deck was weathered and not readily accessible due to rust.

A lifejacket complying with the requirements of paragraph 2.2.1 of the code shall be provided for every person on board the ship. 6 lifejackets were identified as unserviceable. SOLAS (2004) III / Reg 7.2.1

Emergency steering drills shall take place at least once every three months in order to practice emergency steering procedures. These drills include direct control with the steering gear compartment, the communication procedures with the navigation bridge and, where applicable the operation of an alternative power supplies. Vessel was outside its 3 month window and shall conduct emergency steering drills. SOLAS (14)/V 26.4

Where a three-phase supply is used an alarm shall be provided that will indicate failure of any one of the supply phases. The alarm shall be both audible and visual and shall be situated in a conspicuous position in the main machinery space or control room from which the main machinery is normally controlled. The three-phase supply alarm is inoperable, make alarm operable. SOLAS (04)/II-1 30.3

Ventilation ducts shall be made of non-combustible material. The ventilation duct from the welding station to outside had a connection point secured with duct tape. Secure connection point with non-combustible material. SOLAS (04)/ II-2 9.7.1.1

Update automation test procedures to reflect actual testing procedures, pen and ink changes and water mist system activation. SOLAS (04)/II-2 46.2

The main inlets and outlets of all ventilation systems shall be capable of being closed from outside the spaces being ventilated. The EDG Room outboard Damper did not completely shut. Make damper able to properly close. SOLAS (04)/ II-1 5.2.1.1

Any failure of the oil filtering equipment shall be recorded in the Oil Record Book Part 1. While reviewing the history of the Oil Content Meter, the OWS was run on 16 February 2021 for maintenance due to failure of equipment and not annotated on the Oil Record Book. Annotate all oil filtering equipment failures in the Oil Record Book. MARPOL I/17.5

The condition of the ship and its equiment shall be maintained to conform with the provisions of the present regulations to ensure tha thte ship in all reqpects will remain fit to proceed to sea without danger to the ship or persons on board. An Acetylene bottle was found tied to a drain pipe on the open aft deck, next to full Oxygen locker. Bottle to be moved to a safe and secure location, taking into account fire safety as well as security concerns.

Fire-fighting systems and appliances shall be kept in good working order and readily available for immediate use. The emergency fire pump did not provide the required pressure when initially tested, due to being airbound. Engineers were able to quickly identify and correct the problem and demonstrate proper use of the pump.

The main inlets and outlets of all ventilation systems shall be capable of being closed from outside the spaces being ventilated. The EDG Room Port Side Damper did not completely shut (neither manually nor pneumatically) and handle was broken off. Repair and test to the satisfaction of Class by 14 March.

Container Vessel Safmarine Ngami experienced a loss of propulsion on sailing from Mombasa Container Terminal when the #2 start valve hung open at 2355.

Sludge shore connection pipe found wasted and with soft patch above main deck. Wasted section of the sludge pipe to be repaired to the satisfaction of the attending surveyor.

Soft patch repairs due to pin hole leakages were found on ballast line to Aft Peak Tank in Engine Room Starboard side in between 1st and 2nd platforms, in the following locations: - Frs.29-33 - three (3) patches - Fr.45 - one (1) patch Ballast line to be repaired to surveyor's satisfaction.

A 35cm long x 0.5cm wide crack on frame 56 aft stbd side above the waterline caused by a hard tug hit while coming into Port Qasim, Pakistan on 18 June 2016 at 1906 local time. Crack allowed water ingress into #5 cargo bilge well and caused the bilge alarms to sound several times during the voyage from Port Qasim, Pakistan to Jebel Ali, Dubai, UAE.

Found loose hoses in the CO2 room, ensure CO2 is ready for proper operation.

Generator was not providing the proper voltage output, provide a class report attesting to the proper operation of the #3 Gene.
