In a ship in which oil fuel is used, the arrangments for the storage, distribution and utilization of the oil fuel shall be such as to ensure the safety of the ship and persons on board. PSCO recieved report that forward HFO tank was leaking small quantities of fuel oil into chain locker. PSCO observed sounding tank logs indicating leakage from forward HFRO tank, and oil film on windlass and port hawsepipe. Provide corrective action plan.

The master, owner, operator, agent, or person in charge of a vessel equipped with ballast tanks that operates in the waters of the United States must Install and operate a ballast water management system (BWMS) that has been approved by the Coast Guard under 46 CFR part 162; vessel's Master reported that treatment system failed during transit. Master attested to ballast water exchange using Empty-Refill Method. Vessel is not authorized to discharge ballast water in the U.S. w/out approval from the local COTP.
