The inspection (for certification) is to ensure that the vessel is in satisfactory condition and fit for the service for which it is intended, and that it complies with the applicable statutes and regulations for such vessels. Vessel was unable to complete inspection for certification due to water found in fuel tank that prevented operation of vessel machinery. Items to be made available for inspection:
- Drills
- Hydraulic steering fluid, bilge, general, and fire detection alarms
- Operation of all machinery and steering
- Single station smoke detector in crew's space
- Location of 160B semi-portable fire extinguisher

A vessel must be operated in such a manner as to afford protection against hazards to life, property, and the environment. A significant amount of water was found in vessel's fuel tank which prevented proper operation of machinery. Investigate cause for presence of water, make repairs/update TSMS as necessary.

Resolution: Cause for presence of water investigated, repairs made, TSMS reviewed. Proper operation of MDEs and GENSET witnessed, all sat.
Machinery must be maintained to provide for safe operation of the towing vessel and safety of persons onboard under normal and emergency conditions. The subject vessel was involved in a loss of propulsion of both engines. Provide cause of the event and prove proper operation of the propulsion system prior to returning to service.
