A hatch cover must:(1) Have securing devices; and(2) Be attached to the hatch frame or coaming by hinges, captive chains, or other devices of substantial strength to prevent its loss. Hatch cover on steering and fuel compartment were not attached to the vessel.

The arrangement of the retroreflective material applied after March 11, 1996, must be as specified by IMO Resolution A.658(16). Retroreflective tape needs replaced.
