PSCO observed an air leak in the starboard side lifeboat air support system. Correct prior to carrying cargoes with a flashpoint less than 60 degrees C and require an air supported lifeboat. A chemical tanker shall comply with the requirements of the International Bulk Chemical (IBC) code. Electrical installations shall be such as to minimize the risk of fire and explosion from flammable products. PSCO observed worn conduit connecting to sensor for starboard tank cleaning pipe. Correct prior to carrying cargoes with a flash point less than 60 degrees Celsius. Reference: 74 SOLAS (2024) VII/10 IBC Code 10.1.2.1

Electrical installations shall be such as to minimize the risk of fire and explosion from flammable products. PSCO observed broken bolts on cargo tank 2 port/stbd alarm panels. Correct prior to carrying cargoes with a flash point less than 60 degrees Celsius. Reference: 74 SOLAS (2024) VII/10 IBC Code 10.1.2.1

A chemical tanker shall comply with the requirements of the International Bulk Chemical (IBC) code. Cargo tanks shall be fitted with a visual audible high level alarm which complies with 15.9.1 to 15.9.5 and which indicates when the liquid level in the cargo tank approaches the normal fuel condition. PSCO observed cargo tank 2 stbd high level and overfill alarms to sound at the same level. Correct prior to loading cargo tank in cargo tank 2 stbd. Reference: 74SOLAS (2024) VII/10 IBC Code 15.19.6

Plans and booklets shall be kept up to date; any alterations thereto shall be recorded as soon as practicable. PSCO observed multiple smoke and flame detectors in the vicinity of the auxiliary engines that were not accurately displayed on the fire control plan. 74 SOLAS (2020) II-2/15.2.4.1

0720-Firefighting equipment-Fire Fighting systems and appliances shall be kept in good working order and readily available for immediate use. PSCO discovered 07 failed hoses during operational test of emergency and main fire pump. Vessel received spare and replacement hoses during exam whihc were ordered (2) months prior. 74 SOLAS (14) II-2/14.2.1.2 - 10c

0720-fire fighting equipment-fire protection systems and fire fighting systems and appliances shall be maintained ready for use. PSCO noted ships crew could not get the emergency fire pump to provide pressure while the vessel was in ballast condition. SOLAS 74(a) 2014 cons. II-2/14.1.1. 60a

The vessel's Anchor was reported not operational.

Number four port cargo tank high level alarm does not not alarm on deck. Prior to loading the number four port cargo tank and no later than 30 days from this date, the audible and visual alarm must be operational.
