PSCO observed Main Engine exhaust line lagging in the incinerator space being inadequate, evident by the escape of heat, melted paint, melted wire insulation, and heavy soot.

Oil filtering equipment shall be such of a design approved by the administration and shall be such as will ensure that aby oily mixture discharged into the sea after passing through the system has an Oil Content not exceeding 15ppm. In considering the design of such equipment, the Administration shall have regard to the specifications recommended by the Organization. During the operational test of the OWS, the manual valve was in the isolation position, preventing the effluent from entering the OCM. OCM was not displaying true representative of the sample effluent prior to discharge.
